Class Updates: All class updates will be done through email, Face Book or newsletter. If in the event of a cancelling of class due to weather an email will be sent out and Facebook will be updated. I consent to all email communication from Dance With Miss Liz and understand that email is the main form of communication from Dance With Miss Liz.
Photography and video release: I understand that images of the participant may be used in Dance With Miss Liz ads, promotional videos, website material, or various other marketing. These images will be used for Dance With Miss Liz purposes only, and will not be given or sold to outside companies or individuals. Parents will not be notified prior to the use of a photo taken by, or for Dance With Miss Liz for promotional purposes.

Release and Acceptance of Terms and Conditions
I have carefully read the Terms and Conditions, which explains in detail, how Dance With Miss Liz operates, and by signing, I agree, and will abide by the Rules, Terms and Conditions and decisions of the Dance Director. I realize that dance training and the recreation activities planned by Dance With Miss Liz are physical activities, and may result in injury to my child. I hereby assume all risks in connection with such activities, and release Dance With Miss Liz, their officers, owners and employees from all liability for any injuries or damages whatsoever, and from any claims by me, or my family, estate, heirs or assigns arising in any way from my child's participation in or presence at Dance With Miss Liz.
I have carefully read the foregoing Release and Acceptance of Terms and Conditions, and sign it voluntarily with full knowledge of it's significance.
