Agreement to Create an Account

By creating an account with American Dance Movement LLC (ADM), I confirm that I am 18 years of age or older and am the parent or legal guardian of the dancer(s) I am registering. I understand that I am responsible for providing accurate and current information and for reviewing and agreeing to all ADM policies, guidelines, tuition requirements, and registration terms before enrolling my dancer(s).

American Dance Movement LLC (ADM) – Parent/Student Agreement

I acknowledge that I have read, understand, and agree to abide by all American Dance Movement LLC (ADM) policies, procedures, rules, and guidelines while my child or I are enrolled in the dance school.

I understand that it is my responsibility to review and follow the information provided in the ADM Introduction Packet, which is available through the Parent Portal, in the Files section of the American Dance Movement LLC (ADM) Facebook Parent Group, and on the American Dance Movement (ADM) website. I agree to remain informed of any updates to these policies.

By registering, I also acknowledge and agree to American Dance Movement's LLC (ADM) insurance and liability policies. I understand that participation in dance and related activities involves inherent risks of injury. I voluntarily assume those risks and agree that American Dance Movement LLC (ADM), its owners, instructors, employees, volunteers, and representatives shall not be held liable for any accident, injury, illness, loss, or damage that may occur while attending or participating in any ADM class, rehearsal, performance, competition, convention, event, or travel associated with American Dance Movement LLC, whether on or off the premises, except as otherwise required by applicable law.

American Dance Movement LLC's Policies and Guidelines Handbook provides families with important information about registration, tuition, attendance, dress code, class expectations, studio closures, performances, safety, and parent responsibilities. All dancers and parents are expected to review and follow these guidelines to help create a safe, positive, organized, and enjoyable dance experience for everyone.
