I am fully aware that by allowing myself or my dependent child(ren) to participate in dance instruction, there is a possibility that injuries or illnesses may occur, and I accept responsibility for and assume this risk. I am or my child(ren) are in the proper physical condition and capable of participating in dance instruction and related activities.
I agree to release Olympic Ballroom Dance LLC and their director, owner, students, teachers, staff, employees, volunteers, and associates from any liability or claim that I or my representatives may have against Olympic Ballroom Dance LLC with respect to any bodily injury, personal injury, illness, death, or property loss or damage that may result from my or my child(ren)'s participation at Olympic Ballroom Dance LLC.
I voluntarily release and forever discharge and hold harmless Olympic Ballroom Dance LLC from any and all claims or demands for damages, loss of services, costs and expenses, injuries, attorney fees, and any other call for reparation from any and all injury to me, my child(ren), or my property arising in any way from my or my child(ren)'s participation in dance classes, camps, workshops, performances, the use of equipment or facilities, and any activities associated with Olympic Ballroom Dance LLC.
I authorize and agree that Olympic Ballroom Dance LLC may take and use photographs, videos or likenesses of myself or my child for use in business and/or promotional materials, such as flyers, posters, website and social media posts and advertisements. I convey full rights and interest in these photos and recordings to Olympic Ballroom Dance LLC.
By checking the box, I certify that I am at least 18 years of age and am providing my digital signature that I have read the Waiver and have agreed to its terms. I understand and give consent that checking the box is providing my signature in digital form and shall be treated as a valid and original signature in all respects.
