I, the parent or legal guardian of the student(s) listed above, hereby give approval of the student(s)’s participation in any and all of the programs and activities, including, but not limited to the classes listed above. I waive, release, absolve, indemnify, & agree to hold harmless the organizers, sponsors, supervisors, participants and persons involved in the operation of these programs, activities, and classes for any claims arising out of injury or other loss to named applicant or any member of his/her family whether as a participant or spectator.
I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by The Studio and Studio K Dance. I/we agree to release and hold harmless The Studio and Studio K Dance including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold The Studio or Studio K Dance liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by The Studio and Studio K Dance.
I acknowledge and consent to allow The Studio and Studio K Dance to use: photos and/or videos of my child/myself in publications (such as flyers/brochures), advertisements (such as newspapers or online ads), our studio’s website, or on social media sites (such as the studio’s facebook or instagram account). I understand that in some cases, my child and/or my name may be used. I further understand no financial compensation will be for use of these photos/videos.
PRODUCTION FEES: The Production fee helps cover the auditorium rental and janitor fees for out June Recital. This $75 fee (per family) must be paid in full by January 1st. NEW FOR 2026-2027 No Complimentary tickets will be provided for families. Tickets must be purchased online and will be reserved seating.
COSTUME FEES: All dancers will be responsible for purchasing costumes for the Spring Recital. Costume prices are as follows: Primary Levels- $70. Diamond, Ruby, Bronze, Silver Levels- $90 Emerald, Sapphire, Amethyst, Gold, Platinum - $90 Teachers will measure students after Commitment week, but costumes will not be ordered until all costumes payments have been paid. All Costume fees must be paid for by November 25th, or you may pay in installments through the Fall session. All costumes will include a pair of tights, garment bag and hanger.
2027 Recital Week Dates June 9th-June 12th. I understand that if I wish for my student to participate in Recital, they must be available to attend all rehearsals of Recital week. Tech Rehearsal June 9th 5pm-9pm Dress Rehearsal June 10th 5pm-9pm Recital Show #1 June 11th 4:45pm-9pm Recital Show #2 June 12th 12:45pm-5pm
All students at The Studio are welcomed and encouraged to participate in the Spring Recital. However, should a student choose not to participate in the Spring Production, he/she is still welcome to attend classes. COMMITMENT: Commitment week is the first week of November. If you do not wish to participate in the recital, you must contact the office on Recital Commitment Week to be removed from the recital list and have your costume and production fees removed from your account. Once you have committed your child to performing in the Spring Recital, you will be responsible for the Production and Costume Fees.
