As parent/legal guardian of the student(s), I give my consent my for my child(ren) to participate in the programs at the Spring Lake Community House(SLCH). I understand it is the express intent of the SLCH to provide for the safety of the student(s). I understand that participation in dance and related activities may result in injuries, e.g. torn/strained muscles, broken bones, or other serious injuries. As a parent/ legal guardian, I agree to provide health insurance for the student(s) or guaranteed payment of medical expenses incurred as a result of training, performing, or participating in activities at SLCH. In consideration of using the SLCH facility, I waive all rights or causes of action against the Board of Trustees and/or SLCH for injuries or other damages suffered by my child(ren) and myself while under the supervision of the SLCH. It is also my intent to release the SLCH and its employees from liability in the future. I understand that payment is due in advance of classes and refunds and credits cannot be given for missed classes. I accept the conditions of registration and the liability release. I am signing this voluntarily and over 18 years of age.
