Photo & Media Release

I, the undersigned, grant Miss Michele & Co. permission to take photographs and video recordings of me (or
my child) during classes, rehearsals, and performances. These images may be used for promotional
purposes, including social media, the studio website, advertisements, and print materials.
I understand that no financial compensation will be provided for the use of these images. I release Miss
Michele & Co., its owners, and representatives from any claims related to the use of such images.

Liability Waiver & Assumption of Risk

I acknowledge that dance and related activities involve physical movement and carry a risk of
injury.
By signing below, I voluntarily assume all risks associated with participation in classes,
rehearsals, performances, competitions, conventions, and events organized, sponsored or
attended by Miss Michele & Co, whether held at the studio location or at an off-site venue.

I release and hold harmless Miss Michele & Co., its owners, instructors, staff, and volunteers from
any and all liability, claims, or demands for accidental injuries, illnesses, or damages sustained
during participation, except in cases of gross negligence or willful misconduct.

I certify that I (or my child) am physically fit to participate and that I have disclosed any medical
conditions to the studio.
