I give permission for my child to participate in MDS Dance Classes. I understand that participation in dance involves physical activity and I hereby release and hold harmless Melinda’s Dance Studio, its owners, instructors, and assistants from any and all claims or liability for any injury or accident that may occur during participation in classes, rehearsals, or related activities.

Melinda’s Dance Studio is not responsible for lost or stolen items. Please label all personal belongings.

I also grant permission for my child’s image to be used in photographs or video for studio marketing purposes. No names will be included. If you prefer that your child’s image not be used, please notify the studio in writing.
