* Please ensure you complete and sign all required waivers when creating your account.

* While payment by credit card is not mandatory for lessons, each account is required to have a valid and up-to-date credit or debit card on file.

By registering for classes, workshops, or events at Leslie School of Dance, I acknowledge that I will be required to review and agree to all individual waivers, consent forms, and policies applicable to my participation.

I understand that my registration is not complete until I have signed these individual waivers, and I agree to comply with all terms outlined in those documents.

I release Leslie School of Dance from liability related to participation, as detailed in the individual waivers.

Students may withdraw from classes at any time with at least two weeks’ written notice prior to the withdrawal date.

To officially withdraw from lessons, a parent, guardian, or adult student must submit a withdrawal notice via email. Verbal withdrawals or non-attendance do not qualify as official notice.

By registering (unless for a trial or drop-in class), I understand that I am committing to a full dance season. If my dancer chooses not to continue with a class during the season, I agree to send a written withdrawal notice by email. I understand that I am responsible for tuition up until the date that the written notice is received, including the two-week notice period.

The full season consists of:

Two 12-week semesters

One 6-week half semester (for those participating in the recital)

At Leslie School of Dance, participation in our annual dance recital is a fun and rewarding experience—but it is not mandatory.

However, recital planning begins early in the season and involves choreography, costuming, and studio scheduling. For this reason, we will assume that all students are participating in the recital unless we receive an official opt-out notice via email.

Our 2026 Recitals will be held on Saturday, May 30 and Sunday, May 31, 2026.
Dress rehearsals are mandatory and will take place during the week of May 26, 2026. Exact dates and times will be announced closer to the event.

By signing this, I acknowledge and agree to the following:

I understand that recital participation is optional, but my child will be included in recital dances, costume planning, and choreography unless I send an email to opt out.

If I do not opt out by the posted deadline, I am responsible for any recital-related fees, including costume charges, that are incurred on behalf of my child.

If my dancer is participating in the recital, I agree to follow all costume and rehearsal policies and meet all deadlines related to recital participation.

I understand that attendance at dress rehearsal is required in order for my dancer to perform in the recital.

I, the undersigned, acknowledge and understand that participation in dance classes, acro classes, and cirque classes; rehearsals and performances, and any other related activities at Leslie School of Dance involves inherent risks of physical injury. These risks include, but are not limited to, sprains, pulled muscles, fractures, or other injuries.

By enrolling myself or my child, I voluntarily assume all such risks and accept full responsibility for being aware of my and my child’s physical limitations, agreeing not to exceed them during any studio-related activities.

I hereby release, waive, discharge, and hold harmless Leslie School of Dance, its owner, instructors, assistants, dancers, volunteers, guest artists, staff, employees, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage that may be sustained by myself or my child while participating in classes, workshops, rehearsals, performances, or other activities affiliated with the studio.

Emergency Medical Treatment: In the event of an injury or medical emergency during participation in studio activities, I authorize Leslie School of Dance and its representatives to seek medical treatment for myself or my child as deemed necessary. I understand that every effort will be made to contact me or the emergency contacts provided before any treatment is administered.

At Leslie School of Dance, we love celebrating our dancers and sharing their accomplishments. We also respect the privacy and comfort of all families. Please review the following guidelines regarding photo and video use:

As a parent/guardian, I agree to be mindful and respectful when posting photos or videos on social media. I will not post images or footage of other dancers without the consent of their parent or guardian.

I understand that I have the option to opt out of having my child’s photo or video used for promotional purposes by indicating this during registration. If I do not opt out, I give Leslie School of Dance permission to use photos or videos of my child for marketing, social media, website, or printed materials.

If I choose to opt out, I understand that my child may be asked to step out of group photos or videos taken during class or special studio events to respect our documentation policy.

Regardless of my opt-out selection, I understand that:

My child will still appear in the class photo that hangs on the studio wall (unless they do not participate in the end-of-year photo session).

My child may appear in the recital video/DVD, which is shared with families after the performance.

By signing this waiver, I acknowledge and agree to the terms outlined above.

As a member of Leslie School of Dance, I agree to use social media in a respectful and positive manner when representing or referring to the studio, its staff, students, and affiliated families.

I understand and agree to the following:

I will not post negative, harmful, or inappropriate comments about dancers, instructors, staff, families, or any individuals associated with Leslie School of Dance on any social media platforms.

I will refrain from sharing any confidential or sensitive information related to the studio or its members.

I understand that failure to uphold this policy may result in disciplinary action, including removal from classes or events, at the discretion of the studio.

Leslie School of Dance is committed to creating a supportive and respectful community both in-person and online. I agree to contribute to that environment by representing the studio with integrity and kindness across all social platforms.

At Leslie School of Dance, we are committed to providing a safe and supportive environment for all students. To best support your dancer, we ask that parents/guardians disclose any medical conditions, special needs, learning differences, behavioral considerations, or required accommodations that may affect their participation in class.

This includes (but is not limited to):

Allergies or chronic medical conditions

Medications that must be taken during class hours

Physical, developmental, cognitive, or emotional needs

Any support your child receives in a school setting that may be helpful for us to know

By signing below, I acknowledge that I am responsible for informing Leslie School of Dance in writing of any such needs or conditions at the time of registration or as they arise throughout the season. I understand that this information will be kept confidential and used solely to ensure the safety and well-being of my dancer.

Failure to disclose relevant information may limit our ability to provide appropriate support and may result in safety concerns or removal from class.

By signing this agreement, I acknowledge and accept full financial responsibility for all fees associated with the classes I am registering for at Leslie School of Dance, including tuition, costume fees, and any other applicable charges.

I understand the following policies:

Recital Participation: While recital participation is encouraged, it is not required. If I choose not to participate, I must notify the studio in writing (email is acceptable).

Tuition Policy: Tuition is non-refundable once paid. If I wish to withdraw from a class, I must provide at least two weeks’ written notice via email. I am responsible for tuition up to and including that two-week notice period. Failure to formally withdraw will result in continued tuition charges.

Costume Fees: If participating in the recital, I understand that once a costume deposit is made and costumes are ordered, the full costume balance becomes non-refundable and is my responsibility.

Payment Method: Autopay enrollment is required. If tuition is not paid by the due date, the card on file will be automatically charged. To avoid this, I may submit payment by check or cash before the due date.

Payment Options: I may choose to pay tuition in full or in installments. Installment payments can be made using autopay, or manually via check or cash.

I acknowledge that I have reviewed the current tuition rates provided by Leslie School of Dance and agree to the terms outlined above.
