You must be a parent or guardian of a dancer/ prospective dancer to register for an account.

All students and parents are aware of possible physical injury that may occur during
dance classes and are willing to assume those risks. It is agreed that students, and
their parents will not hold Mod Dance LLC liable for injuries sustained while
participating in any ModDance Studio activity. We agree to, and will abide by, all rules
and regulations set forth for any specialty group we register for. We give our
permission to Mod Dance LLC to use pictures and videos of our child for advertising
purposes. I understand that I am responsible for all late fees incurred when any
payments are not made on time.
