<ul><li>I agree to abide by all policies of Flourish Dance Academy.</li>
<li>I understand that Flourish Dance Academy reserves the right to refuse instruction to anyone not abiding by studio policies.</li>
<li>I acknowledge that Flourish Dance Academy is not responsible for lost, stolen, or unclaimed items.</li>
<li>I understand and voluntarily accept the risks associated with participation in dance classes, rehearsals, performances, and related travel, including but not limited to injury, illness, or property damage.</li>
<li>I agree that Flourish Dance Academy and its staff will not be held responsible for injuries or illnesses sustained in class, in rehearsals, while performing, or while traveling to or from its facilities.</li>
<li>I assume full financial responsibility for any personal and bodily injury and/or illness that may occur during participation in Flourish Dance Academy activities.</li>
<li>I certify that I have and will maintain, at my expense, sufficient health and medical insurance to cover any potential costs related to such injury or illness.</li>
<li>I release Flourish Dance Academy and its managers, instructors, employees, agents, affiliates, successors, and assigns from all liability related to injury or illness as described above, including any claims arising from actual or alleged negligence in supervision, instruction, training, or facility care.</li>
<li>I am the parent or legal guardian of the minor participant. I have read and understood this waiver and am authorized to sign on their behalf.</li></ul>
