I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of THE DANCE MOVEMENT LLC. classes, rehearsals, performances, or activities.

I also exempt, release, and indemnify THE DANCE MOVEMENT LLC, it's owners, agents, volunteers, assistants, employees, staff, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by THE DANCE MOVEMENT LLC. In addition, I hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold THE DANCE MOVEMENT LLC., its owners, agents, volunteers, assistants, employees, staff, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death.

I give permission to THE DANCE MOVEMENT LLC. to provide and/or seek medical attention for my child should medical treatment be required and I will assume all costs of my child/children's medical bill(s).

I give THE DANCE MOVEMENT LLC. permission to utilize event photos, videos, and statements without compensation for advertising purposes. Each participant may refuse to participate in any activity. If there are any doubts as to a dancer's physical ability, please consult your physician before participating.

I also understand that there are NO REFUNDS at anytime for any reason. I have read this waiver agreement, under- stand its contents, and accept and assume such risks.

If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights. I have read, understood and agree to be bound by the above statement.

I understand that TUITION is due on the FIRST of each month and is based on a season -regardless of how many weeks are in the month. I understand that if I wish to withdraw from classes at The Dance Movement, I am required to notify the studio in
writing via email and until such time, the space will be held for my student and I am responsible for the charges.

I have read this waiver agreement, under- stand its contents, and accept and assume such risks.

I understand that TUITION is due on the FIRST of each month and is based on a season -regardless of how many weeks are in the month. I understand that if fees remain unpaid in week 4 of classes, dancers will not be permitted to participate in class(es).
I understand that credit card payments can be made online only and ALL digital payments will include a processing fee.

I understand that if I wish to withdraw from classes at The Dance Movement, I am required to notify the studio in writing via email and until such time, the space will be held for my student and I am responsible for the charges.
