RELEASE AND AUTHORIZATIONS

Name of Student: _________________________________________________ has no health problems or conditions of which the dance center should be aware (such as heart, back, medical, allergy, muscular, pregnancy, diabetes, epilepsy, chemical or neurological condition, particular medication, knee/kidney/shoulder problems, etc.). I understand that risk of injury is inherent in any physical activity, and I knowingly and voluntarily accept that risk on behalf of myself and my child. I, the undersigned, for myself, my heirs, administrators, and executors, hereby waive and release Southern Arc Dance Center LLC, its staff, independent contractors, and employees from any claims or damages of any kind arising out of my child’s participation in the exercise and/or dance program of Southern Arc Dance Center LLC. I certify that the student above is in proper physical condition to participate in the exercise/dance program and that he/she has been examined by a licensed physician and found to be in appropriate physical condition to participate in said program. I, the undersigned, do hereby authorize Southern Arc Dance Center, LLC or its designated agents (teachers, Independent contractors, or administrators employed by Southern Arc Dance Center LLC) to obtain medical treatment for my said child in emergencies where I cannot be reached in time to authorize the treating physician to provide such emergency medical services. I understand that I am responsible for any medical expenses and that the absence of health insurance does not make Southern Arc Dance Center LLC accountable for paying medical expenses. This authority includes the power to authorize any necessary treatment under the circumstances by a licensed physician. By sighing below, I acknowledge that I am aware that classes, studio showings, and performances may be photographed and videotaped; I am releasing the registered student’s image so it may be used for promotion, advertising, and archival use. This power is, in essence, a power of attorney and shall remain in effect until revoked by me in writing.

I grant permission to Southern Arc Dance, LLC, and its agents or employees, to use photographs of me or the minor named here for studio publications, including brochures, newspapers, and the Southern Arc Dance Center, LLC social media and Website at www.southernarcdance.org.
I grant Southern Arc Dance the unrestricted right to use and publish photograph or video footage taken of me/and my minor child while participating in dance class, performing, or in the company of Southern Arc Dance Center, LLC, I waive any right to royalties or other compensation arising from the use of my photograph, and if singing on behalf of a minor named herein, I waive all such rights of the child named herein.
I agree on my behalf or the minor child named herein to release and hold harmless Southern Arc Dance Center, LLC, and its agents, Independent contractors, or employees from any claims arising from the related use of the photographs. I acknowledge that by signing this form, I give Southern Arc Dance full copyright and authority to publish photography and agree to the above-mentioned terms.
For the Student:
As a Student of Southern Arc Dance Center, LLC, I understand that I am held to a high standard of conduct. I will respect my instructors and classmates. I will be on time to class and dress and conduct myself appropriately in the center/class.

Tuition and Fees
Monthly Tuition: Required for all students, determined by the number of weekly instructional hours chosen.
Registration Fee: A one-time, non-refundable $50.00 fee, along with the first months tuition, is due at registration.
Credit Card Requirements: A valid credit card must be on file to register for classes.
Billing Schedule: Tuition is automatically billed on the 1st of each month.
Dance Season: Runs for ten (10) months from August to May, based on at least 32 classes per season. The final tuition payment is made on May 1st each season.
End-of-Season Show: Usually held on the first Saturday of June.
Performance Fee: A $95.00 end-of-season performance fee per student is due in February.
· Costume Fees: $100.00 per costume, posted to accounts in November.
NSF Fee: A $40.00 fee for any declined card payment will be charged.
Policies
Automatic Payments: All tuition is paid monthly via automatic credit card processing. Accounts are charged on the 1st day of each month for ten consecutive months from August to May. Timely payment is essential to avoid late fees.
No Refunds: Southern Arc Dance, Center LLC, and Southern Arc Dance Theater, Inc. do not offer refunds for any fees, including Registration, Costumes, Performance Fees, Tuition, Tickets, Late Fees, or any miscellaneous fees.
Class Withdrawals: Class withdrawals require a 30-day notice. For example, if you withdraw on November 5th, you will still be charged for December 1st tuition.
Missed Classes: No Makeup classes are provided for missed classes.
School Calendar: We follow the Coweta County School System calendar for closed dates, except for a few teacher workdays.

STUDENT, LIABILITY WAIVER, AND MEDIA RELEASE

WAIVER OF LIABILITY, ASSUMPTION OF RISK, RELEASE, AND INDEMNIFICATION AGREEMENT
I, the undersigned Parent or Legal Guardian, affirm that I am eighteen (18) years of age or older and legally authorized to execute this agreement on behalf of the minor child named above.
I understand that participation in dance classes, rehearsals, performances, conditioning exercises, workshops, camps, and related activities offered by Southern Arc Dance Center, LLC and/or Southern Arc Dance Theater, Inc. involves inherent risks of injury, including but not limited to sprains, strains, fractures, falls, physical contact with other participants, and other injuries that may occur during physical activity.
I represent that my child is physically capable of participating in dance and movement activities and that I am not aware of any medical condition that would prevent safe participation. I agree to notify Southern Arc Dance Center, LLC and/or Southern Arc Dance Theater, Inc. of any medical condition, injury, or limitation that may affect participation.
I knowingly and voluntarily assume all risks associated with my child's participation in any activity conducted by Southern Arc Dance Center, LLC and Southern Arc Dance Theater, Inc., whether such risks are known or unknown.
To the fullest extent permitted by applicable law, I release, waive, discharge, and hold harmless Southern Arc Dance Center, LLC, Southern Arc Dance Theater, Inc., their directors, officers, employees, independent contractors, instructors, volunteers, agents, and representatives from any and all claims, demands, actions, damages, losses, liabilities, costs, or expenses arising out of or related to participation in any program, class, rehearsal, performance, event, or activity.
I further agree to indemnify and hold harmless Southern Arc Dance Center, LLC and Southern Arc Dance Theater, Inc. against any claims brought on behalf of or by the participant arising from participation in studio activities.
MEDICAL AUTHORIZATION
In the event of an emergency, I authorize Southern Arc Dance Center, LLC and Southern Arc Dance Theater, Inc. staff, instructors, or representatives to seek emergency medical treatment for my child if I cannot be reached immediately.
I understand that I am solely responsible for all medical expenses incurred on behalf of my child.
PHOTO, VIDEO, AND MEDIA RELEASE
I grant Southern Arc Dance Center, LLC and Southern Arc Dance Theater, Inc., their employees, agents, contractors, and representatives permission to photograph, videotape, record, or otherwise capture images or likenesses of my child during classes, rehearsals, performances, events, workshops, and other activities.
I understand these images, recordings, and videos may be used for:
Marketing and advertising
Social media platforms
Website content
Print publications
News media
Promotional materials
Historical and archival purposes
Grant applications and reports
I grant Southern Arc Dance Center, LLC and Southern Arc Dance Theater, Inc. the unrestricted right to use, reproduce, distribute, publish, display, and archive such images and recordings without compensation or further approval.
I waive any right to royalties, compensation, inspection, or approval of the finished product or its use.
I release and hold harmless Southern Arc Dance Center, LLC and Southern Arc Dance Theater, Inc. from any claims arising from the use of photographs, video recordings, or other media containing my child's image or likeness.
ACKNOWLEDGEMENT
I have carefully read this Registration, Waiver of Liability, Assumption of Risk, Release, Indemnification Agreement, Medical Authorization, and Media Release.
I fully understand its contents and voluntarily agree to all terms contained herein.
I understand that by signing this document, I am waiving certain legal rights on behalf of myself and my minor child.
