Five678 kindly requests that you enroll in a class upon creating your account. In the event that you have questions about classes please contact us at admin@five678dance.com. All account holders must be over 18 to register a dancer(s) for classes at Five678 Studio of Dance.

Liability Waiver, Release & Acknowledgement

All participants and students must read, sign, and agree to this waiver prior to participating in any class, rehearsal, workshop, or event at Five678 Studio of Dance.
If the participant is under the age of eighteen (18), a parent or legal guardian must sign on their behalf.

Admission to class and/or rehearsal will not be permitted unless this form is fully completed and properly signed.

WAIVER OF LIABILITY & ASSUMPTION OF RISK

I acknowledge and understand that participation in dance and dance training involves inherent risks of physical injury, including but not limited to muscle strains, sprains, falls, and other injuries. I voluntarily assume all such risks on behalf of myself and/or my child.

I hereby release, waive, and discharge Five678 Studio of Dance, LLC, its owners, officers, directors, instructors, staff, independent contractors, volunteers, event sponsors, and landlords from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, or damages sustained while participating in or attending any dance classes, rehearsals, workshops, performances, private lessons, events, or activities sponsored by Five678 Studio of Dance.

I further agree to indemnify and hold harmless Five678 Studio of Dance, LLC from any liability, costs, damages, or judgments resulting from acts or omissions by myself or my child that cause injury or damage to another person or property.

PROTECTION OF PERSONAL PROPERTY

I understand that Five678 Studio of Dance is not responsible for lost, stolen, or damaged personal property. I agree that I am solely responsible for safeguarding my personal belongings while on the premises or attending any studio-related activities.

I hereby release Five678 Studio of Dance, LLC and its representatives from all liability related to the loss or damage of personal property.

MEDICAL AUTHORIZATION

In the event of a medical emergency or injury, I authorize a representative of Five678 Studio of Dance to obtain emergency medical treatment and/or transportation for myself or my child as deemed necessary.

I understand that all medical treatment and related expenses will be my sole responsibility. I further understand that reasonable efforts will be made to contact the emergency contact(s) listed on file as soon as possible.

MEDIA RELEASE

I grant permission for Five678 Studio of Dance to use photographs and video recordings of myself or my child taken during classes, rehearsals, performances, workshops, or studio events for instructional, promotional, advertising, and marketing purposes.

Parents or students who do not wish to grant this permission must notify Five678 Studio of Dance in writing prior to participation.

ACKNOWLEDGEMENT OF WAIVER & HANDBOOK

By signing below, I acknowledge that I have read, understand, and agree to the terms of this Liability Waiver and the Five678 Studio of Dance Parent Handbook, including all studio policies and procedures.

I confirm that:

I am at least eighteen (18) years of age and legally authorized to sign this agreement.

I am signing this waiver voluntarily and of my own free will.

Neither I nor my child has any medical condition that would prevent safe participation in dance activities, and participation has been approved by our family physician.

I accept full financial responsibility for any medical expenses incurred due to injury sustained during participation.

This waiver is binding upon myself, my child, and our respective heirs, legal representatives, and assigns.
