AGREEMENT. The parties to this Registration Agreement (from now on referred to as, the “Agreement†agree to be legally bound by and comply with the terms of this Agreement is between, Studio/PraiseTEAM Praise Theatre Ensemble and Arts Ministry Address 10461 140th Street, Surrey, B.C. V3T 4L6 referred to as the “Studio†and/or “PraiseTEAMâ€
CLASS ENROLLMENT. Students who wish to enroll in a class must complete a class selection enrollment form (Schedule A & B) or complete the online registration form process provided by the Studio. The “Registrant†must register & sign for students under the age of 19.
CLASS CANCELLATION. Classes, which do not meet its minimum number of registered students may be canceled. Registrants and/or Students will be notified via phone or email if a class is canceled.
PLACEMENT,INSTRUCTORAPPROVALorAUDITIONBASEDCLASSES.
• Students who wish to register for Placement or Audition based classes that require Instructor Approval does not automatically confirm their acceptance into the class.
• Students must attend an Audition or Placement class session prior to the start of the regular season.
• Students will be contacted via phone or email to confirm their acceptance in the class after attending an Audition or placement class.
PRE-REQUISITES AND CO-REQUISITES. Some classes require students to have attended a certain technique or
fundamental classes as a pre-requisite and/or be enrolled in another class as a Co-requisite.
• Students must be enrolled in the suggested Co-requisite classes concurrently with their other classes throughout the season.
• Students are permitted to enroll in co-requisite classes at another studio. Approval by PraiseTEAM administration is required and a letter of enrollment from the other studio must be submitted to confirm the student’s acceptance in the class.
• Students who wish to enroll in a Co-requisite class that requires Placement, Instructor Approval, or Audition must receive an acceptance confirmation notice prior to attending a class. (See 4b for regulations regarding enrollment policies)
COMMITMENT. Students must commit to scheduled classes, rehearsals, competitions, and/or performances.
Students may not compete for 2 or more separate studios/organizations or a combination of independent and studio/organization in the same level, category, subject, style, or genre at the same competition. Some considerations may apply upon the approval of the Head of the faculty
REGISTRATION FEE. The Registrant agrees to pay to the Studio a one-time registration fee in the amount as specified on our rate sheet.
TUITION FEES. The Registrant will pay to the Studio in advance or on the first day of each month the total amount as per class/classes enrolled and the total amount calculated from the online registration.
Tuition is based on four classes per month. Some months will have fewer classes due to school breaks and closure. Other months have five weeks of classes, depending on the calendar year. Please note that the tuition is not pro-rated when there are fewer than four classes, and in return, there are no added fees when a fifth week occurs. Tuition and other studio fees information provided is subject to change. Most classes will receive approximately 32 classes/year as this is what our tuition fees are based on, from September till June annually.
The Studio follows the British Columbia School District holiday/break and school closure schedule and will be closed on major holidays. The Studio may also be closed due to poor weather conditions. In the event of a school closure, the Studio will do it’s best to notify all Students and/or Registrants via phone or email and the tuition amount will not change for months with school closures due to holidays, breaks, and poor weather conditions.
PAYMENT OF TUITION FEES. Tuition Fees must be received in full by the Studio on or before the first calendar day of each month unless the parties agree in writing in advance to the 15th of the month option. A complete calendar year is from September to June and tuition is due each month until the calendar year is completed
COMPETITION FEES. Students enrolled in a competitive class are required to participate in all competitions determined by the instructor. Competition fees are in addition to tuition fees and must be paid in full by November 30thof the current
season. An approximate amount of $125 will be collected from each member of the competitive group as a deposit until competition organizers have determined their exact fee amount. Any unused portion will be refunded
The Studio reserves the right to refuse the Student from attending classes until the full balance has been settled.
If the student does not wish to participate in competitions, they are allowed to transfer to a non-competitive class provided that they give written notice on or before December 30th of the current season.
This notice must be in writing and must include the full name of the Student, the class they are withdrawing from, the new class they wish to transfer to, be signed and dated by the Student if over 19 years of age or by the Registrant for Students under 19 yrs of age.
There will be no refunds or credits for competition fees once a class is fully registered for competition.
ARREARS. Late payment, returned or non-sufficient funds (NSF) cheques are subject to an administrative fee of $20.00 each, plus the amount of any service fees charged by a financial institution to the Studio. Although these fees are payable by the Registrant to the Studio, failure to pay the tuition fee on the due date is a breach of a material term of this Agreement. The obligation of the Registrant under this Agreement requires the tuition to be paid on the due date. The Studio reserves the right to refuse the Student from attending classes until the full balance has been settled.
CLASS WITHDRAWALS. A student may withdraw from a class at least one calendar month’s written notice. A notice given before the day the tuition is due in a given month ends the registration at the end of the following month. For example, if the student wants to withdraw at the end of November, the Student must make sure the studio receives written notice on or before October 31st. Similarly, if the Student gives notice on the 15thof October, the withdrawal is deemed to end on November 30th, not November 15th.
This notice must be in writing and must include the full name of the student who wishes to withdraw, the date the registration is to end, the class name he/she is withdrawing from, be signed and dated by the Student if over 19 years of age or by the Registrant for Students under 19 yrs of age and include the specific grounds for ending their registration, if the Registrant is withdrawing because the Studio has breached a material term of this Agreement.
All class withdrawals are subject to one month’s tuition fee following the last month of attendance regardless of the number of sessions attended.
WITHDRAWAL CANCELLATIONS. Registrants or Students may rescind a withdrawal notice provided that they notify the studio at least one day’s written notice before the date the withdrawal notice takes into effect.
The Studio reserves the right to refuse the Registrant from attending classes until the rescind notice has been received.
MISSED AND MAKEUP CLASSES. There will be no refunds or credits for missed classes. Students who are unable to attend classes due to illness must inform the studio in writing at least 12 hours prior to the start of the class.
Where the Students must be absent due to illness and 12-hour advance notice is not possible, lessons may be rescheduled. Classes will be made up at the instructor’s discretion. However, if this occurs too often and the instructor feels that this privilege is being abused, the Studio reserves the right to charge the Student the instructor’s private lesson rate.
The make-up of missed classes is permitted subject to the instructor’s availability and may be charged the additional instructor’s private lesson rates. Students are expected to make their own makeup class arrangements with the help of their peers or by requesting a makeup class with their instructor.
With notice and due diligence, instructors will work with students and parents to resolve any unforeseen circumstances that may arise so long as this does not negatively affect other members.
NOTICES, NEWS & UPDATES. Studio notices, news, and updates will be sent by email at least once a month. It is advisable for all Registrants and Students to sign up for the monthly newsletter if they wish to stay informed on all Studio activities. A valid email address must be provided in Schedule A in order to sign up for the Studio’s monthly newsletter. A copy of our monthly newsletter may be found in our social media pages such as Facebook, Instagram &/or Twitter.
ACCIDENT WAIVER AND RELEASE OF LIABILITY. Due to the nature of our program, all Registrants release PraiseTEAM, Praise Theatre Ensemble and Arts Ministry, its legal representatives or assignees, all persons acting under its permission or authority, and all rental facilities being used to conduct classes and/or organizations, including but not limited to the Archdiocese of Vancouver, OLGC Church and the Salesians of Don Bosco (SDB) from all liabilities, claims, actions, damages, and losses of any nature whatsoever which may arise from participation in classes or events with the Studio. PraiseTEAM assumes no liability for damage or theft of personal property.
PHOTO AND MEDIA RELEASE WAIVER. All Registrants agree to the release of any photos or videos taken of the Students for marketing media releases publicity, and display deemed fit by the Studio. Registrants and Students agree to get permission from the studio director, prior to posting any studio related videos on-line or in social media.
16. PERFORMANCEPOLICY.AllStudentsarewelcomeandencouragedtoparticipateintheStudio’smid-yearandyear-end shows however regular class attendance is required. In consideration to the rest of the class, students with poor attendance or missing vital classes or rehearsals leading up may not be permitted to participate or may perform in a limited capacity. Students who are unable or do not wish to perform at the Studio’s mid-year and year-end shows must inform their instructor in writing at least three (3) calendar months prior to the show date.
REGISTRANT AND STUDENT OBLIGATIONS. PraiseTEAM is a Christian Community that welcomes diversity. Registrants and Students are expected to show respect to the Salesians of Don Bosco and Studio personnel, instructors, administration, and fellow students, regardless of age, race, denomination, etc. and will not use foul language, steal, bully, or anything that would hurt their peers physically, psychologically or emotionally.
The Registrant must accept any notice; order, process, or document required or permitted to be given when served.
THE PARTIES, INTENDING TO BE LEGALLY BOUND, AGREE TO THE TERMS AND CONDITIONS OF THIS AGREEMENT. THE REGISTRANT HEREBY ACKNOWLEDGES HAVING READ AND RECEIVED A DUPLICATE COPY OF THIS AGREEMENT.
