Student Waiver (updated Sept 2025)
Since participation in any physical activity includes some element of risk, I release the owner/ director/teachers of Florida Dance Workshop from any and all liability of loss, or personal injury that may occur at the Workshop facility, or at any performance venue. If I cannot be reached, in the event of a medical emergency, I give permission for emergency medical treatment.
I understand that photos and recordings of participants become the property of Florida Dance Workshop and its director and may, therefore, be used for future publicity.
REGISTRATION FEE NOTICE
I understand that a $25.00 registration fee is due upon registration. I also understand that payment is due the on the 1st of each month.
TUITION PAYMENTS NOTICE
I understand that Prepay or Auto Payment is required for Tuition payments. If you wish to pay by check or cash, payments must be received 1 week BEFORE the 1st of
the month. Convenience fees are NOT charged for online payments.
MISCELLANEOUS FEE NOTICE
I understand that Costumes, concession, dancewear and other non tuition charges must be paid by cash, check, credit card within 30 days of purchase or by due date on statement. These items WILL NOT come out on AUTO PAY. Payments more than 90 days late will be sent to collections.
COSTUME FEE NOTICE
I understand that my Showtime costume fee of $85-105.00 will be posted in October and is due by December 1st. Costume payments are due immediately upon registration if the date is after December 1st of that dance season. For students who join after January: the student's ability to participate in the show is dependent on the ability to obtain a costume for the class. Please discuss this with the staff as soon as possible.
LATE FEE NOTICE
I understand that payments are considered late as of the 10th of every month and will occur a
$10.00 late fee for every 30 days past due. LATE FEES ARE NOT REFUNDABLE.
OVATIONS & DANCE COMPANY COSTUMES / ENTRY & CHOREOGRAPHY FEE NOTICE
I understand choreography fees are added in August and are due September 1st. I understand Ovations & Dance Company Costume Payments are added in August and are due October 1. I understand the Convention Fee will be added in August is due October 1st. I understand my entry fees will be posted by the October 1, November 1st and December 1st and are due November 1st, December 1st and January 1st. (Not including any extra competitions like Encore's 4th and Nationals) (approximately 8 weeks prior to the event) and payment is required for my dancer to attend. I understand a $10 late fee will apply to late payments and is not refundable.

I understand that when I register, I am registering for the entire season (Regular or Summer- dates as listed) and will be registered unless I (the parent) contact the staff to unenroll my student from the classes by emailing floridadanceworkshop@gmail or speaking to the front desk.

I further understand that membership requires following those policies posted in the Parent Portal, including given written notice of discontinuation of classes.
