I understand that participation in dance classes, rehearsals, performances, competitions, camps, private lessons, birthday parties, studio events, and any other activities offered by Hit the Floor Studio of Dance Ltd. involves physical activity and may include risks such as slips, falls, muscle strain, injury, illness, or other accidents.
I acknowledge that I am voluntarily registering myself and/or my child to participate in activities at Hit the Floor Studio of Dance Ltd.

I confirm that the participant is physically able to take part in dance and movement activities. I agree to inform the studio of any medical conditions, allergies, injuries, physical limitations, behavioural concerns, or other important information that may affect participation.

I understand that Hit the Floor Studio of Dance Ltd., its owners, directors, instructors, assistants, volunteers, staff, contractors, and representatives will take reasonable care to provide a safe and supportive environment. However, I agree that participation is at my own risk and/or my child’s risk.

I hereby release and hold harmless Hit the Floor Studio of Dance Ltd., its owners, directors, instructors, assistants, volunteers, staff, contractors, and representatives from any claims, demands, losses, damages, costs, or expenses arising from participation in studio activities, except where caused by gross negligence or intentional misconduct.

In the event of an emergency, I authorize Hit the Floor Studio of Dance Ltd. staff to contact emergency services and/or seek medical assistance for the participant if deemed necessary. I understand that I am responsible for any medical or emergency costs incurred.

I understand that proper dance attire and footwear may be required for safety and participation. I agree that the participant will follow studio rules, instructor directions, and respectful behaviour expectations while attending classes, rehearsals, performances, competitions, camps, parties, or events.

I understand that photos and videos may occasionally be taken during classes, performances, competitions, camps, parties, or studio events. These may be used for studio promotional purposes, including social media, website content, printed materials, and advertising. If I do not wish for myself or my child to be photographed or recorded for promotional use, I will notify the studio in writing.

I understand that enrolment, payment, refund, attendance, costume, competition, and studio policies are the responsibility of the registering parent/guardian or adult participant to review and follow.
By completing registration, I confirm that I have read, understood, and agree to the terms of this waiver and release.
