Liability Waiver, Assumption of Risk & Media Consent Agreement
The Parent/Guardian understands and acknowledges that participation in ballet and other forms of dance involves physical activity and carries an inherent risk of injury. By enrolling in programs at Kristina Pulcini Ballet Academy LLC, the Parent/Guardian voluntarily assumes all such risks on behalf of their child and waives all liability against Kristina Pulcini Ballet Academy LLC, its owners, employees, agents, and assigns.
The Parent/Guardian acknowledges that brief physical contact may be used by instructors to safely guide alignment and correct form, and accepts this as a standard part of dance training.
The Parent/Guardian waives all liability for injuries or illnesses that may occur during participation in classes, rehearsals, performances, or other activities held by or under the direction of Kristina Pulcini Ballet Academy LLC, whether on-site or off-site.
In the event of a medical emergency, the Parent/Guardian authorizes Kristina Pulcini Ballet Academy staff to call 911 and consents to emergency treatment for their child. The Parent/Guardian agrees to be responsible for all resulting medical expenses.
The Parent/Guardian acknowledges that in the event in-person classes are temporarily suspended or unavailable due to health concerns, government mandates, or other unforeseen circumstances, KP Ballet may transfer instruction to virtual online classes.
The Parent/Guardian accepts responsibility for any property damage caused by their child and agrees to indemnify Kristina Pulcini Ballet Academy LLC for any such losses.
Unless the Parent/Guardian notifies the Academy in writing prior to the start of the school year, they grant Kristina Pulcini Ballet Academy LLC permission to photograph or film their child during classes, rehearsals, and performances, and to use such media in promotional materials, on the Academy’s website, and across print and digital platforms.
Please note: Because performances are professionally filmed and photographed, participation in performances requires media consent.
The Parent/Guardian and student agree not to post or share photographs or videos of other students or of KP Ballet performances on public platforms.
The Parent/Guardian agrees to abide by all Kristina Pulcini Ballet Academy LLC policies and procedures, as outlined in registration materials and/or published on the KP Ballet website.

Electronic Signature Agreement: By agreeing to this waiver, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement. I acknowledge that I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
