WAIVER AND RELEASE OF ALL CLAIMS AND CONSENT FOR EMERGENCY
MEDICAL TREATMENT FOR PARTICIPANT
Each student(s), individually or through their parent or guardian, recognizes the possibility of injuries or illness resulting from their participation at Forever Dance Alaska, LLC and/or any other activities associated with Forever Dance Alaska, LLC, including transportation to and from each activity.
With that in mind, it is the intent of the undersigned, by executing this document, to waive, release, and hold harmless Forever Dance Alaska, LLC and/or any other activities associated with Forever Dance Alaska, LLC and its staff, and instructors from any liability for any injuries, illnesses or damages sustained by the student, arising out of, in connection with, or resulting from their participation in any activity or function sponsored by Forever Dance Alaska, LLC.
Further, if the student(s) is/are a minor, the undersigned hereby consents to emergency medical treatment, hospitalization, or other medical treatments as needed for the welfare of the student(s) by a physician, qualified nurse, and/or hospital in the event of injury or illness while participating in the event.
In that regard, as parent/guardian of the student(s), I release Forever Dance Alaska, LLC and its staff and instructors from any liability arising out of, in connection with, or resulting from such medical treatment.
I, the parent/guardian of the registrant(s), a minor, agree to allow pictures, written text, and videos to be published in newsletters, newspapers, television, social media, internet, or promotions of Forever Dance Alaska, LLC.
I understand the season start and end dates minus scheduled and holiday closures.
I understand that session fees are calculated on a seasonal basis (9 months). Tuition is divided into 9 payments for your convenience. Tuition is due on the 1st of each month, and a late fee of $20 is applied if payment is not received by the 10th. The parent/guardian is responsible for the FULL session amount and may not be deducted due to illness, injury, vacation, absence, or weather. Tuition may be paid in full at any point. Tuition paid is non-refundable.
A minimum of 30 days’ written notice is required to withdraw from a class. Tuition paid for the current month is non-refundable, regardless of the date the withdrawal notice is submitted. Withdrawal requests are subject to approval by Forever Dance Alaska. Once the 30-day notice has been received and approved, no additional tuition will accrue beyond the applicable 30-day notice period.
I understand that if using homeschool allotment, contract agreements for homeschool VDP are the responsibility of the parent/guardian to be completed before class starts, and tuition remains the parent/guardian responsibility.
I understand that Forever Dance Alaska, LLC registration fee and tuition are non-refundable. No exceptions.
I understand all tuition and all fees must be paid in full to perform at any shows. I understand shows will depend upon the status of KPBSD theater, and Forever Dance Alaska, LLC will communicate this information as it becomes available. Purchased tickets are required to view performances.
I understand that Christmas costumes are rentals. One Christmas costume is included in the registration fee. Any additional performing classes have a $25 costume rental fee per class due October 1. The Christmas costume rental agreement is required and is a separate waiver and must be completed upon registration via the portal.
I understand there will be a Spring costume down payment of $75 applied for each performing class due on October 1. Spring costume balances will be due on February 1. Costume fees are the parent/guardian’s responsibility.
I understand it is the parent/guardian’s responsibility to pay tuition and other fees at the studio front desk, or online through your parent portal.
It is the parent/guardian’s responsibility to keep student information and telephone numbers up to date for Forever Dance Alaska, LLC.
I understand the telephone number, 907-262-1641, and email, info@foreverdancealaska.com, are for contacts and updates. I also know that www.foreverdancealaska.com is the studio website.
I understand that choreography learned at Forever Dance Alaska, LLC cannot be used without the written permission of the Instructor or Director.
I understand that Forever Dance Alaska, LLC is not responsible for lost or stolen items in or out of the building. I also understand that Forever Dance Alaska, LLC is not responsible for car accidents, lost or stolen items in the parking lot or surrounding area.
I understand that my child(ren) can only be dropped off 10-15 minutes before class time and must be picked up within 15 minutes after class time without prior arrangements and permission from the front desk.
Regarding all the above information, by signing, the parent or guardian acknowledges they have read and agree to the information.
