You must be 18 years old or over to register for an account.

I hereby certify that I/my child is in good health and capable of participating in Portland Ballet's classes. I hereby agree to let the PB Office know of any medical conditions that may impact class safety and participation. I hereby release and forever discharge Portland Ballet of and from any and all claims, demands, rights or cause of action of whatsoever kind or nature, arising from, or by reason of, any and all known or unknown, foreseen or unforeseen bodily or personal injuries and illnesses and the consequences thereof.

I hereby agree that Portland Ballet has permission to use photographs and video footage of me/my child for marketing purposes, including print, online, TV, and others. Photos may capture your/your child's participation, directly or indirectly. Dancer names or personal information will not be shared in conjunction with photos/video. In the event that Portland Ballet would like to list a dancer’s name associated with an image or video, permission will be requested prior to publishing.

I hereby agree that Portland Ballet has permission to use my/my child's contact information for Class purposes such as, but are not limited to: Notification of class changes, cancellations due to extenuating circumstances, production schedules, announcements, etc. Contact information will never be given out to a third party without consent.

If a dancer wishes to withdraw from a class, the withdrawal date will be determined by when the Office is notified in writing to office@portlandballet.org; tuition is due until that time. Credits or refunds will be issued following the guidelines listed below:

- Withdrawing before class begins up until the first day of class: A full credit will be issued to your account for a future class OR a refund less a 15% cancellation fee will be processed to the original form of payment. (If withdrawing from class 1+ month before it begins, cancellation fee is waived.)

- Withdrawing within the first 2 weeks of class: Pro-rated credit issued to use for a future class. No refunds issued.

- Withdrawing after 2 weeks into the class session: No credit or refund issued.
