I have read through the studio's policies.
As or parent/guardian of a student enrolled at Create Dance Center, Ltd, I acknowledge that there are certain risks of personal injury. I agree to voluntarily assume those risks and responsibilities, which I, or my child may sustain as a result of participating in any and all activities connected with or associated with Create Dance Center, Ltd.
In the event of an emergency, I understand that I will be contacted immediately and in the interim, I authorize Create Dance Center, staff to secure from any licensed hospital, physician and/or medical personnel any treatment deemed necessary for my child’s immediate care.
By signing this document I am assuming the financial responsibility for my child to participate in a program at Create Dance Center Ltd. I understand that monthly installments of tuition will be auto-drafted by the 11th of each month. If paying by cash/check/zelle, payment must be prior to the 10th. I understand that I must have a valid credit card on file. I understand the costume payments and when they are due.
In a situation of joint custody, the parent/guardian on this contract will be the parent/guardian Create Dance Center Ltd. will contact regarding payments etc.
Should the student need to withdraw, a written document must be completed and turned in before the start of the month to avoid being required to pay further tuition payments.
Create Dance Center, Ltd. reserves the right at any time to the termination of any student who fails to comply with the policies and rules of the studio. In the event of termination of enrollment, any deposit shall be forfeited.
-Photo Policy- I understand that photos and videos of students may be used for advertisement purposes. The names of the students will not be used unless otherwise notified, and additional permission is granted. If I do NOT wish to have my child be photographed/videoed I will submit a written request to the director.
