I, _________________________________________ (print parent name), hereby release Brooklyn Dance Academy and any of its agents, directors, and staff from any and all claims for damages, injuries or loss to myself or my child's person or property during attendance at the school or any of its related functions.

Brooklyn Dance Academy, including its agents and staff, are released from all liability for injury, damage or loss, which may be caused by any act or omission of any of them.

I also understand that students must be picked up directly after their class and if the student leaves the studio, for any reason, Brooklyn Dance Academy & its staff, will not be held responsible. Students are encouraged to wait inside for parents to pick them up, but Brooklyn Dance Academy is not responsible if a student leaves without permission.

Lastly, I understand the risk of attending in-person classes and that Brooklyn Dance Academy nor any of their teachers are responsible in the event that my dancer or my self contract Covid-19 virus. I agree that I am entering the facility at my own risk and in good faith that I also do not feel ill or have symptoms of the Covid-19 virus.

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19:

The novel coronavirus/COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

Brooklyn Dance Academy has put in place preventative measures to reduce the spread off COVID-19; however, Brooklyn Dance Academy cannot guarantee you will not become infected with COVID-19. Further, attending Brooklyn Dance Academy could increase your risk and your child(ren)'s risk of contacting COVID-19.

By accepting this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my children and I may be exposed to or infected by COVID-19 by attending Brooklyn Dance Academy Classes and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Brooklyn Dance Academy may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, volunteers, and program participants and their families.

I voluntarily agree to assume all of the forgoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at Brooklyn Dance Academy. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the studio, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the studio, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in my studio program.
