I ____________________________, the enrolled participant and/or parent/guardian of the participant, agree and understand that Dance is a potentially hazardous activity. I recognize that there are risks inherent in dance training including, but not limited to serious injury. The participant and/or parent/guardian of the participant hereby agrees to participate in the activities of The Edge Dance Studio, and hereby agrees to indemnify and hold harmless The Edge Dance Studio, as well as the instructors, officers, directors, employees, and owners as well as any performance facility against any liability resulting from any injury that may occur to the participant or guest while participating in activities of The Edge Dance Studio or any other venue where class, rehearsals, or performances are held.
The Edge Dance Studio is a “Hands On” dance studio. Please note that Instructors of The Edge Dance Studio and other master class teachers that The Edge Dance Studio hires may provide physical adjustments to dancers’ positions if necessary to assist the dancer in learning the correct position or movement.
The participant and/or parent/guardian of the participant authorize any representative of The Edge Dance Studio to have the participant treated in any medical emergency during their participation in activities of The Edge Dance Studio. Further, the participant and/or parent/guardian of the participant agree to pay all costs associated with medical care and transportation of the participant.

Pictures or videos taken during performances, classes, and rehearsals may be used for publicity & marketing purposes in newspapers, brochures, website, advertisements etc.

Any special medical/health conditions of which the staff should be aware of should be disclosed to the front desk in writing at time of registration.

I HAVE CAREFULLY READ THE ABOVE LIABILITY RELEASE AND SIGN IT WITH FULL KNOWLEDGE OF ITS CONTENT AND SIGNIFICANCE. I HEREBY RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE THE PARTIES LISTED IN PARAGRAPH ONE OF THIS AGREEMENT.

Despite our careful attention to sterilization, disinfection, and use of personal barriers, there is still a chance that you could be exposed to an illness in the studio, just as you might be at your gym, grocery store or favorite restaurant. Social distancing has reduced the transmission of Coronavirus. Although we have taken several measures to provide social distancing and contactless participation, please know that participation in The Edge instructional classes is at your own risk.

Although exposure is unlikely, by enrolling you do accept the risk and consent to participation.

All fees paid are non refundable. Refunds will not be offered for any fees, including but not limited to registration, tuition, recital fees, and costume fees, under any circumstance.
