Acknowledgment of Risk, Responsibility, and Agreement to Studio Policies
As a student (or parent/guardian of a student) at Laura’s Dance Dynamics, I fully understand and acknowledge that participation in dance classes and related activities carries an inherent risk of physical injury. I voluntarily assume full responsibility for any and all injuries, damages, or losses that may occur as a result of participation, whether in class, rehearsal, performance, or any studio-related event, regardless of location.

I hereby release, waive, and hold harmless Laura’s Dance Dynamics, its owners, instructors, staff, affiliates, and volunteers from any and all liability, claims, demands, or causes of action related to such injuries or losses, including those arising from negligence.

Additionally, I agree to obtain and review the Studio Policies and Information Sheet provided at the front desk on my first day of class. I acknowledge that it is my responsibility to read, understand, and adhere to all studio rules, expectations, and procedures as outlined. My continued participation in classes confirms my agreement to comply with all current and future studio policies.

LAURA’S DANCE DYNAMICS
PARTICIPATION AGREEMENT, LIABILITY WAIVER, ASSUMPTION OF RISK & MEDIA RELEASE

This Participation Agreement, Liability Waiver, Assumption of Risk & Media Release (“Agreement”) is entered into by and between Laura’s Dance Dynamics and the undersigned parent/legal guardian on behalf of the participant.
By signing this Agreement, the parent/legal guardian acknowledges and agrees to the following terms and conditions:

1. ACKNOWLEDGMENT OF RISK
I understand that participation in dance instruction, rehearsals, performances, competitions, conventions, tumbling, acro, conditioning, stretching, camps, choreography sessions, private lessons, travel activities, recital activities, backstage participation, and any related activities at Laura’s Dance Dynamics involves inherent physical risks.

These risks may include, but are not limited to:
Slips, trips, falls, and collisions
Sprains, strains, bruises, fractures, dislocations, or concussions
Head, neck, spinal, or orthopedic injuries
Injuries caused by physical exertion, repetitive movement, equipment, flooring, or interaction with other participants
Illness, allergic reactions, communicable illnesses, medical emergencies, permanent disability, emotional distress, or death
I understand that injuries may occur despite careful supervision and safety procedures.

I voluntarily choose to allow my child to participate fully understanding these risks.

2. ASSUMPTION OF RISK
I knowingly and voluntarily assume all risks, both known and unknown, associated with participation in any activity connected to Laura’s Dance Dynamics, including:

Classes
Rehearsals
Competitions
Conventions
Camps
Recitals and performances
Travel-related activities
Backstage participation
Waiting areas, parking lots, sidewalks, and studio premises
I accept full responsibility for any injury, illness, damages, losses, or death that may result from participation.

3. RELEASE & WAIVER OF LIABILITY
To the fullest extent permitted by law, I release, waive, discharge, and covenant not to sue Laura’s Dance Dynamics, including its:

Owners
Directors
Employees
Teachers
Choreographers
Staff members
Volunteers
Independent contractors
Assistants
Representatives
Affiliates
from any and all claims, liabilities, demands, damages, expenses, actions, causes of action, or losses arising from or related to participation in studio activities, including claims based on negligence.

This release includes, but is not limited to:
Personal injury
Property damage
Emotional distress
Illness
Disability
Medical expenses
Loss of income
Wrongful death claims
I understand that this waiver is intended to be as broad and inclusive as permitted under the laws of the State of Texas.

4. MEDICAL AUTHORIZATION
In the event of an emergency, accident, or illness, I authorize Laura’s Dance Dynamics staff to secure emergency medical treatment for my child if I cannot be reached immediately.

I understand:
The studio does not provide medical insurance
Any medical expenses incurred are solely my responsibility
Emergency response decisions may need to be made quickly in the best interest of the participant

5. PHYSICAL FITNESS & HEALTH ACKNOWLEDGMENT
I certify that my child is physically able to safely participate in dance and related activities.

I agree to disclose any:
Medical conditions
Injuries
Allergies
Restrictions
Concussions
Physical limitations
that could affect participation.
I understand that failure to disclose medical information may increase risk to my child.

6. CONCUSSION & INJURY ACKNOWLEDGMENT
I understand that dance, acro, tumbling, and performance activities may involve risk of concussion or other serious injuries.

If my child sustains an injury, concussion, suspected concussion, or medical condition affecting safe participation:

Laura’s Dance Dynamics reserves the right to remove the dancer from activity immediately
Medical clearance may be required before returning
Return-to-participation decisions are at the discretion of studio administration
I understand that safety decisions made by the studio are final.

7. TRAVEL & COMPETITION RELEASE
I understand that participation in conventions, competitions, performances, nationals, off-site rehearsals, and studio travel activities involves additional risks.

I voluntarily authorize my child to participate in off-site activities and release Laura’s Dance Dynamics from liability related to:

Transportation
Hotels
Convention centers
Competition venues
Public facilities
Third-party event organizers
I understand that parents/guardians remain responsible for their child outside of designated studio supervision times.

8. PREMISES & PICK-UP ACKNOWLEDGMENT
I understand:

Dance activities involve active movement and crowded environments
Parking lots, backstage areas, dressing rooms, stairs, hallways, and waiting areas may present hazards
I am responsible for supervising siblings and non-participating children
Laura’s Dance Dynamics is not responsible for:

Lost or stolen items
Personal belongings
Injuries occurring outside supervised activity times
I agree to follow all studio drop-off, pick-up, backstage, recital, and safety procedures.

9. PHOTO, VIDEO & SOCIAL MEDIA RELEASE
I grant permission for Laura’s Dance Dynamics to photograph and/or record my child during classes, rehearsals, competitions, performances, camps, and events.

I authorize the use of photographs, videos, livestreams, and recordings for:
Social media
Advertising
Promotional materials
Website content
Printed materials
Studio displays
Recital videos
without compensation or further approval.

10. STUDIO POLICIES & SAFETY AUTHORITY
I understand that Laura’s Dance Dynamics reserves the right to:

Remove any participant for unsafe behavior
Enforce dress code and safety policies
Restrict participation due to injury, illness, conduct, or non-compliance
Refuse participation if studio policies are violated
I agree to comply with all studio rules, policies, procedures, and staff instructions.

11. SEVERABILITY
If any portion of this Agreement is determined to be invalid or unenforceable, the remaining portions shall remain in full force and effect.

12. ACKNOWLEDGMENT & SIGNATURE
I certify that:

I am the parent/legal guardian of the participant listed below
I have carefully read and fully understand this Agreement
I understand I am giving up substantial legal rights on behalf of myself and my child
I sign this Agreement voluntarily and without coercion

STUDIO POLICIES ACKNOWLEDGMENT

I acknowledge that I have been informed of and given access to the current studio policies, handbook, procedures, schedules, tuition information, recital information, safety policies, and financial policies of Laura’s Dance Dynamics.
I understand that it is my responsibility to read, review, and remain informed of all studio policies and communications distributed through email, the parent portal, handbooks, newsletters, posted notices, Band, social media, and/or studio communication platforms.
I understand that a copy of studio policies may be requested at any time and that updated policies may be issued throughout the dance season as needed.

By signing this Agreement, I acknowledge that I have read, understand, and agree to comply with all studio policies, procedures, financial obligations, and participation expectations established by Laura’s Dance Dynamics.

By enrolling at Laura’s Dance Dynamics, I acknowledge that I have been informed of the important events, dates, policies, and associated fees that are part of the 2026-2027 Dance Season.

Program Dates
Fall Classes Begin: August 17, 2026
Winter Show: Saturday, December 5, 2026
Spring Recital: Friday, May 21, 2027 & Saturday, May 22, 2027
(Sunday, May 23, 2027 pending if needed)
Season Ends: May 23, 2027

All dates, venues, and events are subject to change.

Winter Show
Winter Show participation is assumed as part of the dance season experience for all students unless written notice of non-participation is submitted to the front office by the designated deadline. Participation includes a performance fee that will be billed separately through Auto-Pay.

Spring Recital
Spring Recital participation is assumed as part of the dance season experience for all students unless written notice of non-participation is submitted to the front office by the designated deadline. Participation includes a recital fee and costume cost per class. Recital Bundles and costume fees will be billed through Auto-Pay.

Costumes will only be ordered once two-thirds (2/3) of the total costume balance has been paid. All recital fees, costume payments, and performance-related charges are non-refundable once processed or once costumes/materials have been ordered.

Payment Authorization & Financial Responsibility
By signing this agreement, I authorize Laura’s Dance Dynamics to charge the payment method on file for tuition, performance fees, recital bundles, costumes, and any outstanding dance-related balances according to studio policies.

Any unpaid tuition or dance-related fees remaining after May 30, 2027, will be charged to the card on file to finalize the account.

Withdrawal from classes does not automatically remove financial responsibility for previously incurred tuition, costume fees, recital fees, competition fees, or performance-related charges.

Accounts must remain in good standing for dancers to participate in performances, receive costumes, awards, videos, photos, or other end-of-season materials.

By signing this form, I confirm that I have read and understand the expectations for participation, event dates, policies, and financial commitments associated with the 2026-2027 Dance Season at Laura’s Dance Dynamics.
