I understand that costume and competition fees (if applicable) will be added to your account after registration. It is my responsibility to ensure these fees are paid in full by November 1, 2025. I understand that competition fees are non-refundable after October 1, 2025, regardless of withdrawal, injury, or other circumstances.
I hereby waive any claims to injury or loss to person or property during participation in classes and virtual/online classes, rehearsals, performances, or any other Studio West activities, and release, discharge and save harmless promoters, directors, principals, agents, staff and instructors from any liability for injury, damage or loss to property or person whatsoever, which may be caused by an act or omission of these.
I, the parent or guardian indicated by name in this registration, understand that I am taking sole responsibility for the tuition, registration and competition/costume fees associated with the classes I have registered my child in. I am also acknowledging that I will receive a payment registration form in my registration completion email and it is my responsibility to return this completed form to the studio within one week of registering. Failure to return this form does not negate my responsibility for payment of fees at a later date. If for some reason, automatic payments are not withdrawn, I agree to notify the studio right away as I am still responsible for these payments. I agree to enroll in auto-pay as a form of automatic payment but acknowledge Studio West Dance Inc. uses multiple processors which include Dance Studio Pro, Square and Stripe. There will be a $10 fee for each declined payment. The original declined amount along with a $25 NSF charge will be reattempted within 10 days after the date of the initial payment. If there is still a balance owing after 10 days you will be charged an additional $20 late fee. If the late fee and balance owing are not paid by the last day of the month, your child may not be able to attend their class(es) until resolved. Please contact us to discuss options if needed.
I hereby grant my child permission to be photographed or filmed (by teachers or parents) while participating in classes or competitions with Studio West Dance. I also grant permission for my child’s photograph or video to be used for promotional / instructional material including the Studio West website, social media or brochure. By agreeing to the waiver, I hereby state that I am the legal guardian of said child. Child Protection Policy I understand that I do not have the express consent to record, film or take photos of children who are not my own unless they have given direct consent. I understand that I am not allowed to post photos or videos online of any child other than my own unless express consent is given by that child and/or parent.
Parents and students are prohibited from contacting teachers through their personal social media accounts, personal phones or emails. Our teachers and staff have the right to a peaceful work/life balance and should not feel the need to answer or communicate with parents or students via their personal devices. It’s important that all communication go through studio channels. While we strive to be readily available to each of our families, we are not available 24/7. Every staff member at SWD should be afforded the right to a work/life balance which means we will respond to requests within an appropriate amount of time during working hours. Emails and phone calls will not be responded to outside working office hours. We appreciate your support in this.
Dance Season Withdrawal Policy (2025/26) Dance season tuition is based on a 10-month program. The final date for withdrawal for our recreational program is before November 1, 2025. Written notice of withdrawal must be submitted to the office. - If received before November 1, tuition will be prorated based on classes already taken, and future charges will be canceled. - No refunds will be issued on/after November 1, including for tuition, competitions, and costumes. - Withdrawals due to injury will be accommodated upon receipt of a doctor’s note, with tuition stopped the following month. Company Program Withdrawal - Deadline to withdraw from a company program or class is before October 1st. Written notice of withdrawal must be submitted to the office. Musical Production Withdrawal Policy - Elf Jr.: Last withdrawal date is September 30, 2025 - Descendants Jr.: Last withdrawal date is January 15, 2026 If you withdraw before these dates, you will be refunded 75% of your fee. After these dates, no refunds will be issued. Camp Withdrawal Policy Camp fees are based on a five-day program. The deadline for camp withdrawal is 14 business days prior to the start date. A written withdrawal request must be submitted. After this date, fees and deposits will not be returned. Withdrawals due to injury will be refunded with a doctor’s note.
