LIABILITY WAIVER & RELEASE AGREEMENT
Savage Sirens All-Star Cheer Co. LLC
I, the parent/legal guardian of the above-named athlete, understand that participation in cheerleading,
tumbling, stunting, and related activities involves inherent risks, including but not limited to serious
injury, permanent disability, or death.
I voluntarily allow my child to participate in all activities offered by Savage Sirens All-Star Cheer Co.
LLC and acknowledge the following:
• I understand the risks associated with cheerleading and accept full responsibility for any injury that
may occur.
• I release and hold harmless Savage Sirens All-Star Cheer Co. LLC, its owners, coaches, staff, and
facility from any and all liability, claims, or demands arising from participation.
• I confirm that my child is physically able to participate and has no medical conditions that would
prevent safe involvement.
• I agree to inform staff of any injuries, medical conditions, or limitations prior to participation.
• In the event of an emergency, I authorize staff to seek medical treatment for my child if I cannot be
reached.

ACKNOWLEDGEMENT:
By agreeing below, I acknowledge that I have read and fully understand this waiver and agree to all
terms.

PHOTO AND VIDEO RELEASE AUTHORIZATION

Authorization for Use of Photographs, Videos, and Likeness

1. GRANT OF PERMISSION
I hereby grant permission to the facility, its owners, employees, and authorized representatives to take photographs, video recordings, and other visual or audio recordings of me and/or my child during participation in classes, competitions, performances, and other activities.

2. AUTHORIZED USES
I authorize the use of these images and recordings for the following purposes:
*Website and social media marketing
*Promotional materials (brochures, flyers, advertisements)
*Training and instructional materials
*News and media coverage
*Competition and event documentation
*Internal facility communications

3. NO COMPENSATION
I understand and agree that no monetary compensation will be provided for the use of these images or recordings. I waive any right to royalties or other compensation arising from or related to the use of these materials.

4. OWNERSHIP
I acknowledge that all photographs, videos, and recordings taken by the facility or its representatives are the property of the facility. I release any claim of ownership or copyright to these materials.

5. DURATION
This authorization shall remain in effect unless revoked in writing. I understand that I may revoke this authorization at any time by providing written notice to the facility, but such revocation will not affect materials already produced or distributed.

6. RELEASE OF LIABILITY
I release and hold harmless the facility, its owners, employees, and representatives from any and all claims, demands, or causes of action arising from the use of photographs, videos, or recordings as authorized herein.

OPT-OUT OPTION
If you do NOT wish to grant photo/video permission, please notify the front desk in writing. Note that we cannot guarantee exclusion from group photos or videos taken during public events or competitions.

By accepting below, I confirm that I have read and understand this Photo and Video Release Authorization and voluntarily consent to its terms.
