MVMNTUM Dance Center - Liability Waiver & Studio Policies

ASSUMPTION OF RISK AND RELEASE OF LIABILITY
I understand that dance, movement, and physical activity carry an inherent risk of injury. I am enrolling my student in classes at MVMNTUM Dance Center with full knowledge of this risk. In consideration of being allowed to participate, I release MVMNTUM Dance Center, its owners, instructors, and staff from any liability for injury, loss, or damage arising from participation in classes, rehearsals, performances, or studio events, except in cases of gross negligence.

TUITION AND ATTENDANCE
Tuition is billed monthly based on the classes and schedule selected at enrollment. I understand that consistent attendance is part of a student's progress, and that missed classes may be made up according to the studio's make-up class policy but are not refunded or credited.

REGISTRATION AND TRIAL FEES
I understand that the registration fee and any trial class fee are one-time charges that are non-refundable, and that they do not count toward monthly tuition.

PHOTO AND MEDIA RELEASE
From time to time, MVMNTUM Dance Center photographs and records students during class, rehearsals, and performances for use on our website, social media, and marketing materials. By accepting this policy, I give permission for my student's image to be used in this way. If you prefer that your student not be photographed or recorded, please notify the studio in writing and we will honor that request.

ACKNOWLEDGEMENT
By accepting this waiver, I confirm that I have read and agree to the policies above on behalf of myself and my student.
