Master Participation Waiver, Policy Agreement, and Release Form
1. Policies and Procedures Agreement (Mandatory)
Acknowledge & Obey: I certify that I have received, read, and fully understand the Infinite Edge Movement (IEM) Policy Manual. This includes the Safe Progression Policy, Dress Code, Drop-off/Pick-up Protocol, and Health/Illness Guidelines.
Compliance: I agree that my child and I will strictly follow all facility house rules, safety policies, and coach instructions.
Enforcement: I understand that failure to follow these policies may result in my child's temporary suspension or permanent removal from classes without a refund.

2. Participation Risk Acknowledgement
Physical Limits: I acknowledge that acrobatics, tumbling, and aerial arts are high-intensity activities. They test a person’s physical and mental limits.
Inherent Risks: I understand that these activities carry potential risks. These include property loss, serious bodily injury, chronic strain, or death.
Risk Sources: I acknowledge these risks may come from facility conditions, equipment wear, weather, or the actions of other participants, coaches, and spectators. I voluntarily choose to allow my child to participate despite these risks.

3. Accident Waiver, Release of Liability, and Indemnity
Assumption of Risk: I hereby assume all physical and financial risks for my child participating at Infinite Edge Movement.
Personal Responsibility: I certify that any injuries or damages caused to my child, or caused by my child to others due to misuse of equipment, breaking of house rules, or dangerous horseplay, are solely my responsibility.
Release of Claims: I release, hold harmless, and indemnify Infinite Edge Movement, its owners, managers, coaches, and landlords from all liability, loss, cost, claims, or damages. This includes claims based on standard negligence.
Legal Scope: This waiver and release of liability shall be constructed broadly to provide a release to the maximum extent permissible under applicable local laws.

4. Emergency Medical Consent and Expenses
Treatment Authorization: I hereby give consent to receive medical treatment deemed advisable by the staff in charge in the event of an injury, accident, or sudden illness during studio hours.
Financial Liability: I explicitly agree to pay all costs associated with emergency medical treatment. This includes ambulance transport, doctor fees, and hospital charges. I will not hold IEM liable for any medical bills.

5. Photography, Video, and Media Terms
Promotional Consent: I give permission for my child to be photographed and videoed by authorized IEM staff for promotional purposes, including the official IEM website and social media platforms.
Media Opt-Out: I understand that if I wish to withhold media consent, I must submit an official Media Opt-Out Form to the front desk before classes begin.

Privacy Restriction: In the interest of protecting student privacy and child safety laws, parents and visitors are strictly prohibited from filming or photographing classes, choreography, or other minor students without prior written consent from management. Any breach of this rule will entitle IEM to immediately terminate this enrolment agreement.
