I confirm that I am the parent or legal guardian of the student(s) being registered and that I am at least 18 years of age.

Please review the following cancellation, withdrawal, and refund policies carefully. By enrolling in a Blueprint Dance Company program, the parent or legal guardian acknowledges and agrees to the terms outlined below.

1. Family-Initiated Withdrawal

A 30-day written notice submitted by email is required to withdraw a dancer from a Blueprint Dance Company program.

Tuition and account balances remain the responsibility of the family during the 30-day notice period and must be paid in full prior to withdrawal.

A withdrawal request is not considered complete until written notice has been received and processed by Blueprint Dance Company.

2. Tuition Following Withdrawal

Families remain responsible for all tuition and other charges incurred during the required 30-day notice period.

If a tuition payment has already been processed when a withdrawal request is submitted, that payment remains non-refundable unless an exception is approved by Blueprint Dance Company.

3. Refunds

Registration fees are non-refundable once paid. Registration fees are separate from tuition and are not applied toward future tuition payments.

Tuition payments are non-refundable once processed, except when Blueprint Dance Company determines an exception is appropriate or when a program is canceled by Blueprint Dance Company.

Blueprint Dance Company does not provide refunds or credits for missed classes, vacations, illness, schedule conflicts, or voluntary withdrawal during the applicable notice period.

4. Missed Classes

Tuition is based on enrollment in the program and is not based on individual class attendance.

No refund or credit will be issued when a dancer misses a scheduled class due to illness, vacation, personal scheduling conflicts, or other reasons.

5. Blueprint Dance Company Cancellations

If Blueprint Dance Company must cancel a scheduled class, Blueprint Dance Company may offer a make-up class or schedule adjustment when feasible. Any make-up class or schedule adjustment is subject to the availability and approval of the host facility, as well as Blueprint Dance Company's scheduling availability.

6. Minimum Enrollment & Program Launch

Blueprint Dance Company programs are subject to minimum enrollment requirements. If a class does not meet the required minimum enrollment within two weeks of registration opening and cannot launch, families who have registered and paid will receive a full refund of tuition and registration fees paid for that class.

Once a class has launched, a subsequent change in enrollment does not qualify for a refund under this provision.

7. Weather & Facility Closures

Programs will typically follow the closure decisions of the host facility and/or the DeKalb County School District when applicable.

Families will be notified of schedule changes or cancellations.

When possible, Blueprint Dance Company may offer make-up classes or schedule adjustments for extended closures or missed instructional time, in coordination with the host facility.

8. Program Changes

Blueprint Dance Company may make reasonable changes to class schedules, instructors, or program arrangements when necessary to operate the program.

Families will be notified of significant changes whenever reasonably possible.

9. Withdrawal for Nonpayment

Withdrawal due to an unpaid balance is governed by the Blueprint Dance Company Payment & Tuition Policy.

Any outstanding balance remains the responsibility of the family. Re-enrollment following withdrawal for nonpayment is subject to the applicable $50 reinstatement fee, resolution of the outstanding balance, and program availability.

10. Parent/Guardian Acknowledgment

By accepting this policy, I acknowledge that:

I understand that 30 days' written notice by email is required to withdraw my dancer from a Blueprint Dance Company program.
I understand that registration fees are non-refundable once paid, including if I decide not to participate in the program after completing registration.
I understand that tuition and account balances remain my responsibility during the 30-day notice period.
I understand that tuition payments are non-refundable once processed, except as otherwise provided in this policy.
I understand that missed classes do not result in automatic refunds or credits.
I understand that Blueprint Dance Company may offer make-up classes or schedule adjustments when a class is canceled or missed due to certain circumstances, when feasible and subject to applicable facility and scheduling availability.
I understand that weather and facility closures may affect the program schedule and that Blueprint Dance Company will communicate applicable changes.
I understand that withdrawal does not eliminate any outstanding balance owed to Blueprint Dance Company.
I understand that withdrawal for nonpayment and subsequent re-enrollment are subject to the Blueprint Dance Company Payment & Tuition Policy.

Blueprint Dance Company is committed to providing a positive, respectful, inclusive, and safe learning environment where dancers can learn, grow, and enjoy dance. This Code of Conduct establishes expectations for dancers and their parents or legal guardians participating in Blueprint Dance Company programs.

1. Dancer Expectations

Dancers are expected to:

Follow instructor directions and classroom expectations.
Treat instructors, classmates, staff, and other participants with respect.
Use appropriate language and behavior.
Participate safely and appropriately for their age and program.
Respect the classroom, equipment, and host facility.
Avoid behavior that intentionally disrupts instruction or creates a safety concern for themselves or others.

Blueprint Dance Company recognizes that behavior should be considered in an age-appropriate manner, particularly for younger dancers. Instructors may provide appropriate reminders, redirection, and support to help dancers meet classroom expectations.

2. Parent & Guardian Expectations

Parents and legal guardians are expected to:

Treat Blueprint Dance Company instructors, staff, contractors, and representatives with respect.
Communicate questions, concerns, or complaints directly with Blueprint Dance Company through appropriate communication channels.
Maintain respectful and professional communication, including in person, by email, by phone, and through electronic communication.
Respect the policies and procedures of the host daycare, school, or educational facility.
Avoid disrupting scheduled instruction or interfering with the instructor's ability to teach.
Follow any applicable arrival, dismissal, pickup, or facility procedures established by the host facility.
Refrain from confronting, disciplining, or addressing another child participating in the program. Concerns involving another dancer should be directed to Blueprint Dance Company or the appropriate host-facility staff.

3. Safety & Disruptive Behavior

Blueprint Dance Company strives to maintain a positive and respectful learning environment. Dancers are expected to follow instructor directions and participate in a manner that supports the safety and learning of themselves and others.

Repeated unsafe, disruptive, disrespectful, or inappropriate behavior may result in temporary removal from class participation.

Blueprint Dance Company may communicate with the parent or legal guardian when behavior concerns arise and may determine appropriate next steps based on the circumstances, the dancer's age, and the nature and severity of the behavior.

4. Communication & Conflict Resolution

Blueprint Dance Company welcomes questions and feedback regarding a dancer's experience.

Parents and legal guardians are encouraged to communicate concerns directly with Blueprint Dance Company and allow reasonable time for the concern to be reviewed and addressed.

Concerns should be communicated respectfully and through appropriate channels. Blueprint Dance Company will make reasonable efforts to address concerns professionally and in the best interest of the dancer and the program.

5. Serious or Repeated Conduct Concerns

Depending on the nature and severity of a situation, Blueprint Dance Company may take one or more of the following actions:

Provide a verbal reminder or redirection.
Communicate with the parent or legal guardian.
Temporarily remove a dancer from class participation.
Require a parent or legal guardian to participate in a discussion regarding continued participation.
Take additional action when necessary to protect the safety, learning environment, or well-being of dancers, instructors, staff, or others.

Serious or repeated conduct that compromises the safety or learning environment will result in removal from the program.

6. No Automatic Refund or Credit

Removal from a class or program due to behavioral or conduct concerns does not automatically entitle the family to a refund, credit, or cancellation of tuition obligations.

Any exception is at the discretion of Blueprint Dance Company and will be considered based on the circumstances.

7. Parent/Guardian Acknowledgment

By accepting this policy, I acknowledge that:

I have read and understand the expectations outlined in the Blueprint Dance Company Parent & Dancer Code of Conduct.
I understand that both dancers and parents/legal guardians are expected to behave respectfully and appropriately.
I understand that Blueprint Dance Company may take appropriate action when behavior creates a safety concern or significantly disrupts the learning environment.
I understand that serious or repeated conduct concerns may result in removal from the program.
I understand that removal due to behavioral or conduct concerns does not automatically result in a refund or credit.
I agree to support Blueprint Dance Company's efforts to maintain a safe, respectful, and positive learning environment.
I agree to the terms of the Blueprint Dance Company Parent & Dancer Code of Conduct.

Please review the following payment and tuition policies carefully. By enrolling in a Blueprint Dance Company program, the parent or legal guardian acknowledges and agrees to the payment terms outlined below.

1. Tuition Due Date

Tuition payments are due by the 5th of each month.

Tuition will be automatically processed using the payment method on file according to the payment schedule established at enrollment.

2. Late Payments & Late Fee

A $25 late fee will be applied to accounts with an unpaid tuition balance after the 5th of the month.

The late fee is applied once per monthly tuition charge and does not replace or reduce the outstanding tuition balance.

3. Past-Due Accounts

Accounts with an outstanding balance after the 10th of the month may result in the dancer being unable to participate in Blueprint Dance Company classes until the account is brought current.

Families will receive email communication regarding overdue balances and any applicable enrollment or participation status updates.

4. Monthly Tuition & Proration

Tuition is charged monthly based on enrollment in the program. Monthly tuition is not prorated based on the enrollment date or the number of classes scheduled in a month.

5. Extended Nonpayment & Withdrawal

Accounts that remain unpaid for 30 days may result in automatic withdrawal from the Blueprint Dance Company program.

Any outstanding balance remains the responsibility of the parent or legal guardian following withdrawal.

6. Reinstatement Following Withdrawal

If a dancer is withdrawn from a Blueprint Dance Company program due to an unpaid balance, re-enrollment is not automatic.

If the family wishes to re-enroll the dancer in a Blueprint Dance Company program, the outstanding balance must be paid in full and a $50 reinstatement fee will apply. Re-enrollment is subject to program availability and completion of any applicable registration requirements.

7. Payment Method

Families are responsible for maintaining a valid payment method on file and ensuring that sufficient funds are available when tuition is processed.

If an automatic payment is unsuccessful, the parent or legal guardian is responsible for updating the payment method or making the outstanding payment promptly.

8. Financial Hardship & Payment Arrangements

Families experiencing financial hardship or needing temporary payment assistance are encouraged to contact Blueprint Dance Company before the tuition due date to discuss whether a temporary payment arrangement may be available.

Payment arrangements are considered on a case-by-case basis and are not guaranteed. An approved payment arrangement must be honored according to the terms agreed upon with Blueprint Dance Company.

9. Parent/Guardian Acknowledgment

By accepting this policy, I acknowledge that:

I understand tuition is due by the 5th of each month.
I understand that tuition may be automatically processed using the payment method on file.
I understand that I may make a manual payment through the Parent Portal before the tuition due date.
I understand that a $25 late fee will be applied to unpaid tuition beginning on the 6th.
I understand the consequences associated with past-due and extended unpaid balances.
I understand that a dancer may be withdrawn from the program due to extended nonpayment.
I understand that a $50 reinstatement fee will apply if my dancer is withdrawn due to an unpaid balance and I later wish to re-enroll.
I am responsible for maintaining accurate payment information and resolving unsuccessful payments promptly.
I understand that any outstanding balance remains my responsibility.
I understand that payment arrangements are considered on a case-by-case basis and must be approved by Blueprint Dance Company.
I agree to the terms of the Blueprint Dance Company Payment & Tuition Policy.

IMPORTANT: PLEASE READ THIS POLICY CAREFULLY BEFORE ACCEPTING.

Blueprint Dance Company may occasionally photograph and/or record dancers during Blueprint Dance Company programs and activities to document, celebrate, and promote the joy of dance and the Blueprint Dance Company experience.

1. Permission to Photograph & Record

By enrolling a dancer in a Blueprint Dance Company program, the parent or legal guardian grants Blueprint Dance Company permission to photograph and/or record the participating dancer during Blueprint Dance Company activities.

Photography and video will be used occasionally and selectively. Content may include individual dancers, groups of dancers, classroom activities, choreography, movement, performances, or other moments that represent the program experience.

Blueprint Dance Company will make reasonable efforts to use professional and appropriate angles, including images or video where a dancer's face is not visible when practical. However, it may not always be possible to avoid showing a dancer's face when capturing group activities, movement, performances, or other program moments.

2. Purpose & Use

Photos and video are intended to showcase dance participation, creativity, movement, student growth, and the positive learning environment provided by Blueprint Dance Company.

Blueprint Dance Company may use photos and video for legitimate business, educational, marketing, and promotional purposes, including:

Social media accounts
Website content
Promotional materials and advertisements
Digital and print marketing
Program announcements
Dance education and instructional content
Other Blueprint Dance Company business-related communications and promotional materials

Blueprint Dance Company will use reasonable judgment when capturing, selecting, editing, and presenting content and will make reasonable efforts to ensure that photos and video are professional, age-appropriate, respectful, and appropriate for public viewing. Content will not intentionally be used in a manner that is inappropriate, degrading, exploitative, or intended to embarrass or misrepresent a dancer.

3. Student Privacy

Blueprint Dance Company will NOT publicly identify a participating dancer by name in connection with photographs or video.

Blueprint Dance Company will also NOT intentionally publicly disclose or identify:

The name of the daycare, school, or educational facility.
The specific program location.
The dancer's address or contact information.
Other personal identifying, sensitive, or family information.

Photos and video will be presented in a manner intended to protect the privacy and dignity of participating dancers while allowing Blueprint Dance Company to share and promote its programs.

4. No Compensation

I understand that I will not receive payment, compensation, royalties, or other consideration for the permitted use of photographs or video recordings.

5. Duration of Permission

This permission applies to photographs and video recorded during the dancer's participation in Blueprint Dance Company programs and remains in effect for the duration of the dancer's enrollment and participation, unless otherwise required by applicable law.

6. Parent/Guardian Acknowledgment

By accepting this policy, I confirm that I have read and understand the Blueprint Dance Company Photo & Video Release and agree to the terms outlined above, including the use of photographs and video for Blueprint Dance Company's business, educational, marketing, and promotional purposes.

I understand that this permission is a condition of enrollment in Blueprint Dance Company programs.
