1. Assumption of Risk
I, the undersigned parent or legal guardian, give permission for my child to participate in activities, rehearsals, workshops, and theatrical performances organized by SF Youtheatre.

I understand that participation in theater activities involves inherent risks, including but not limited to:

Physical exertion during choreography, staging, and vocal exercises.

Potential slip, trip, or fall hazards associated with stage equipment, props, dynamic lighting, sets, and backstage areas.

Standard risks associated with group activities and transportation to/from designated rehearsal or venue locations.

I knowingly and voluntarily assume all risks associated with my child’s participation in SF Youtheatre programs.

2. Waiver and Release of Liability
In consideration for allowing my child to participate in SF Youtheatre, I hereby release, waive, discharge, and hold harmless SF Youtheatre, its officers, directors, instructors, volunteers, staff, independent contractors, partners, and facility owners from any and all claims, demands, causes of action, or liabilities for personal injury, property damage, illness, or accidental loss arising out of or related to my child's participation in any program or event.

3. Medical Authorization & Emergency Care
In the event of an injury, sudden illness, or emergency during an SF Youtheatre event:

I authorize SF Youtheatre staff and designated representatives to administer basic first aid.

I authorize SF Youtheatre staff to seek emergency medical treatment for my child, including transportation via ambulance and care by licensed medical professionals, if I cannot be reached immediately.

I agree to assume full financial responsibility for all costs, medical bills, and expenses incurred for emergency treatment.


4. Photo & Media Release (Optional/Standard)
I hereby grant SF Youtheatre permission to use photographs, video footage, or audio recordings of my child taken during rehearsals, workshops, and performances for organizational materials, including promotional flyers, social media, program booklets, and website updates.

5. Participant Behavior Policy
Participants are expected to follow safety guidelines, respect staff, fellow actors, facilities, and props. SF Youtheatre reserves the right to dismiss any participant whose conduct is unsafe, disruptive, or harmful to others, without refund of tuition or fees.

Acknowledgment of Understanding & Signature
I have carefully read this Liability Waiver, Medical Release, and Photo Authorization and fully understand its contents. I am aware that by signing this document, I am waiving certain legal rights for myself and my child, and I sign it freely and voluntarily.
