Release of Liability & Waiver
For the purposes of this Release of Liability and Waiver, “I/we” refers to the participant and, where the participant is under 18 years of age at the time of registration, their parent or legal guardian.
I/we agree to disclose any relevant medical, developmental, or other health information to Omnes Studios staff prior to the participant’s involvement in any Omnes Studios program. This includes, but is not limited to, developmental disorders (such as ADHD, Autism Spectrum Disorder, and learning disorders), physical disabilities or limitations, existing or new injuries, and other medical conditions (such as asthma or allergies). I/we will also notify staff if the participant uses or may require an inhaler, EpiPen, or other medical device or medication during programming.
I/we agree to monitor the participant for symptoms of common illnesses, including colds and influenza, and to keep the participant home when they are experiencing symptoms. Symptoms may include, but are not limited to, coughing, sore throat, runny nose, congestion, sneezing, fever, vomiting, and diarrhea. If a participant develops or displays symptoms of illness while participating in programming, Omnes Studios reserves the right to remove the participant from the program. In the case of a participant under 18 years of age, their parent or guardian will be contacted and required to pick them up immediately.
I/we acknowledge that participation in studio classes, activities, and other programming may involve the risk of personal injury, illness, or exposure to infectious diseases, including COVID-19. While Omnes Studios takes reasonable precautions to promote a safe environment, accidents, injuries, and illness may still occur. By registering for and participating in Omnes Studios programming, I/we voluntarily assume all risks associated with the use of any premises, facilities, equipment, or spaces used by Omnes Studios.
To the fullest extent permitted by law, I/we agree to release and hold harmless Omnes Studios, its owners, directors, instructors, teachers, dancers, employees, volunteers, staff members, and any facilities or entities used by Omnes Studios from any and all causes of action, claims, demands, or liability, whether known or unknown, arising now or in the future. I/we agree not to hold Omnes Studios responsible for any personal injury, illness, or loss of or damage to personal property that may occur before, during, or after participation in classes, activities, or programming.
I/we understand that photographs, videos, or other recordings may be taken during Omnes Studios programming. Unless Omnes Studios is notified in writing otherwise, I/we consent to the participant being photographed, filmed, or recorded and to the use of their image, likeness, voice, or performance for promotional, marketing, social media, website, advertising, or other business-related purposes without compensation.
I/we agree to follow all applicable class, facility, Studio, and Code of Conduct rules and accept responsibility for my/our behaviour and actions, including any damage I/we may cause to facilities, equipment, or property used by Omnes Studios. I/we acknowledge that if a participant demonstrates repeated or significant behavioural concerns during programming, Omnes Studios reserves the right to remove the participant from the program. In the case of a participant under 18 years of age, their parent or guardian will be contacted and required to pick them up immediately.
I/we understand that Omnes Studios is a licensed and insured organization. If I/we observe any unsafe conduct, conditions, or circumstances before, during, or after participation in programming, I/we agree to report them as soon as possible to an administrator, instructor, or other Omnes Studios staff member.
By submitting registration, I/we confirm that I/we have read, understood, and agreed to the statements above. I/we acknowledge and voluntarily assume all risks associated with participation in Omnes Studios programming and the use of any premises, facilities, equipment, or spaces used by Omnes Studios, and agree to the release of liability outlined above.
