Waiver and Release from Liability & Media Consent

I am participating in, or granting permission for my child, (if under 18), to participate in activities provided by Second Star Theatre Co.

I understand the nature of these activities and acknowledge that, despite precautions, there is a risk of accident or injury.

I agree to release and hold harmless Second Star Theatre Co., its staff, teachers, and partners from any claims, demands, or causes of action arising from participation, including personal injury or property loss, whether occurring before, during, or after any class, rehearsal, performance, or event.

I agree to follow all Second Star Theatre Co. policies and accept responsibility for my/my child’s behavior, including any damage caused to facilities or property.

I grant permission for Second Star Theatre Co. to photograph and/or record me/my child and to use these images or recordings for promotional purposes, including social media, website, and marketing materials, without compensation.
