By agreeing to this Participation Release, I acknowledge that participation in dance classes, rehearsals, performances, camps, workshops, competitions, and other activities offered by JorDance involves physical activity and carries inherent risks.

I certify that I, or my child, am physically able to participate in these activities and understand the importance of following all instructions provided by JorDance instructors and staff. I agree to notify JorDance of any medical conditions, injuries, allergies, or physical limitations that may affect participation.

In consideration of participation in JorDance programs, I voluntarily assume all risks associated with participation and, to the fullest extent permitted by applicable law, release and hold harmless JorDance, its owner, instructors, employees, contractors, volunteers, and affiliated facilities from any and all claims, demands, causes of action, damages, or liabilities arising out of participation in JorDance activities, except to the extent caused by gross negligence or willful misconduct.

In the event of an emergency, I authorize JorDance to obtain emergency medical treatment for myself or my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.

I grant JorDance permission to photograph, record, and film me or my child during classes, performances, rehearsals, competitions, and studio events.

I understand that no compensation will be provided for the use of these materials.
