LIABILITY WAIVER, ASSUMPTION OF RISK, AND MEDIA RELEASE

By registering my child or myself ("Participant") for classes, rehearsals, workshops, examinations, or performances with NrutyaBharati Center for Performing Arts LLC, operating as NBSK/NBSOK ("NBSK," "the Studio"), I agree to the following terms on behalf of the Participant:

1. Assumption of Risk. I understand that Kathak, like all forms of dance, carries inherent risks of injury, including but not limited to: strains, sprains, or blisters from rapid footwork (tatkar) performed barefoot or in ghungroos; dizziness, loss of balance, or falls from spins (chakkars); muscle soreness or fatigue from rehearsal and performance activity; entanglement or abrasion from props, costumes, or ghungroos; incidental contact with other students in shared class space; and general risks of physical exertion. I voluntarily accept these risks on behalf of the Participant.

2. Release of Liability. To the fullest extent permitted by law, I release NBSK, its Artistic Director, instructors, staff, and any venue where NBSK classes or events are held, from any claims or liability for injury, illness, or loss arising from Participant's involvement in NBSK classes, rehearsals, performances, or examinations, whether on-site or off-site, except to the extent caused by gross negligence, recklessness, or intentional misconduct.

3. Medical Authorization. I confirm the Participant is physically able to safely take part in class. I will notify NBSK in writing of any medical condition, injury, or limitation that could affect safe participation. In a medical emergency, I authorize NBSK staff to seek emergency medical treatment for the Participant if I cannot immediately be reached, and I accept responsibility for any resulting medical costs.

4. Conduct & Property. I agree the Participant will follow all studio and venue rules and treat instructors, fellow students, and facility property with respect, and I accept responsibility for any damage caused by the Participant's negligent or intentional conduct.

5. Photo, Video & Media Consent. I consent to NBSK photographing or recording classes, rehearsals, and performances, and using the Participant's name, image, and likeness for NBSK's website, social media, and promotional materials, without compensation. I may withdraw this consent at any time by written notice to NBSK; withdrawal applies going forward only.

6. Curriculum & Intellectual Property. I acknowledge that NBSK's choreography, syllabi, tihais, todas, bandishes, and related recordings are the proprietary work of NBSK and its Artistic Director, provided solely for the Participant's personal instruction, and may not be reproduced, taught to others, or used to support a competing dance program without NBSK's prior written consent.

7. Governing Law. This agreement is governed by the laws of the State of New Jersey. If any provision is found unenforceable, the remaining provisions remain in effect.

8. Acknowledgment. I have read and understood this waiver, understand I am giving up legal rights by agreeing to it, and confirm this on behalf of myself and/or the Participant named in this registration, of my own free will.
