By selecting "I Agree," I acknowledge that I have reviewed and understand the Bloomsburg School of Dance registration information, policies, and agreements. I agree to abide by the studio's policies and understand that it is my responsibility to keep my family's contact information current and to review communications sent through the Parent Portal, email, and other official studio channels.
I understand that completing this registration enrolls my dancer(s) according to the information provided and that I may contact the studio with any questions regarding my account or studio policies.

To help keep our dancers, families, and staff healthy, please do not bring your dancer to class if they are ill.
Students must be:
Fever-free for at least 24 hours without the use of fever-reducing medication before returning to class.
Free from vomiting and/or diarrhea for at least 24 hours before returning to class.
Well enough to participate fully in class activities.
If a student becomes ill during class, a parent or guardian may be contacted to pick them up. Thank you for helping us provide a healthy environment for all of our dance families.

The safety of our students is a top priority at Bloomsburg School of Dance. In the event of an illness, injury, or medical emergency, studio staff will provide appropriate first aid within the scope of their training. If emergency medical care is deemed necessary, 911 will be contacted immediately, followed by the parent/guardian and listed emergency contact(s).
Parents/guardians are responsible for maintaining current emergency contact information, medical conditions, allergies, and medications in the Parent Portal. By registering, you authorize Bloomsburg School of Dance to obtain emergency medical assistance for your dancer if a parent or guardian cannot be reached.

By registering at Bloomsburg School of Dance, I grant permission for my dancer to be photographed and/or recorded during classes, rehearsals, performances, competitions, and studio events. These images and videos may be used for studio promotional purposes, including social media, the website, and printed materials.
If you wish to opt out of photo and video use, you must notify Bloomsburg School of Dance staff in writing through the Parent Portal or by contacting the studio directly. While we will make every reasonable effort to honor your request, we cannot control photos or videos taken by families or spectators at public events.

Bloomsburg School of Dance values your privacy and is committed to protecting your personal information.
The information you provide during registration is used solely for studio operations, including enrollment, billing, scheduling, communication, emergency contact purposes, and providing the best experience for our dance families. We do not sell your personal information. Information is shared only with trusted service providers as necessary to operate the studio, process payments, or comply with legal obligations.
By registering with Bloomsburg School of Dance, you acknowledge that you have read and agree to this Privacy Policy.

understand that participation in dance classes, rehearsals, performances, competitions, and related activities at Bloomsburg School of Dance involves physical activity and carries an inherent risk of injury.
By registering my dancer(s), I voluntarily assume all risks associated with participation and release, waive, and hold harmless Bloomsburg School of Dance, its owners, instructors, staff, and volunteers from any and all claims, liabilities, injuries, damages, or expenses arising from participation, except as prohibited by applicable law.
I certify that my dancer is physically able to participate in dance activities. I understand it is my responsibility to inform the studio of any medical conditions, injuries, or limitations that may affect my dancer's participation.
By completing registration, I acknowledge that I have read, understand, and agree to this Release of Liability.

y completing registration, I acknowledge and agree to the following:
Tuition is charged on a monthly basis and reserves my dancer's place in class for the season. Tuition is not based on the number of classes attended each month.
Monthly tuition is due by the studio's designated due date. Late fees may be applied to past-due accounts.
Tuition is non-refundable and is not reduced for absences, holidays, weather-related closures, or other scheduled studio closures.
I understand that it is my responsibility to notify the studio in writing if my dancer will be withdrawing from classes. Tuition will continue to be charged until written notice of withdrawal is received and processed according to studio policy.
Competition Team tuition and fees are separate from recreational tuition. Multi-class discounts apply only to eligible recreational classes.
I understand that any outstanding balances must be paid in full before costumes, recital participation, competition participation, or other studio activities may be permitted.
I agree to keep my payment information current and understand that I am responsible for all tuition and fees associated with my account.
