Assumption of Risk
I understand that participation in dance classes, rehearsals, performances, camps, workshops, special events, and other activities offered by Roots of Knowledge Academy & Creative Arts Studio LLC involves inherent risks.
These risks include, but are not limited to:
Falls
Slips
Trips
Muscle strains and sprains
Bruises
Cuts and scrapes
Joint injuries
Fractures
Concussions
Injuries resulting from physical activity or interaction with other participants. I voluntarily choose to allow myself and/or my child to participate in these activities and knowingly assume these risks.
Safety
For the safety of all students:
No running inside the studio.
No playing on the stairs or in other rooms.
Please watch siblings or other children you bring with you that may not be participating in class closely as this can be a distraction to other dancers and create a safety concern for all.
Please keep talking to a respectful level.
If you need to take a phone call please do so in a respectful manner by going outside.
Food, drinks (other than water), and gum are not permitted in dance areas. All snacks, and food need to be eaten outside the Bell Tower to be respectful of the space. Any food or drinks other than water will be asked to take outside or discard them immediately.
Students should immediately report injuries or unsafe conditions to an instructor.
Parents should notify instructors of any injuries that may affect participation.
Release of Liability
To the fullest extent permitted by law, I release and hold harmless Roots of Knowledge Academy & Creative Arts Studio LLC, its owners, instructors, assistants, volunteers, independent contractors, landlords, and representatives from claims for injuries, damages, losses, or expenses arising from participation in studio activities, except where caused by gross negligence or willful misconduct.
Medical Authorization
In the event of an illness or emergency and I cannot be reached, I authorize the studio to seek appropriate emergency medical treatment for my child. I understand that I am financially responsible for any medical care received.
Illness Policy
To help maintain a healthy environment for everyone: Students should remain home if they have a fever, vomiting, diarrhea, or other contagious symptoms. Students may return when they are fever-free for at least 24 hours without fever-reducing medication or as recommended by their healthcare provider.
Parent Responsibilities
I agree to:
Ensure tuition and applicable fees are paid on time.
Have my child arrive for class on time 5-10min early to use restroom, hair done, change into dancewear & be ready to dance.
Pick up my child promptly after class.
Keep my contact information current.
Notify the studio of any changes to emergency contacts or medical information.
Treat instructors, staff, volunteers, and other families with kindness and respect.
The studio is not responsible for children before or after scheduled class times unless participating in a supervised studio activity.
Parents or guardians are responsible for ensuring children arrive & leave safely. Parent's may stay in the dance room or partake in other activities happening on site at the Bell Tower, but parent's must remain on site for any child 8 years old or younger.
Student Expectations
Students are expected to:
Follow instructor directions.
Show respect toward classmates and staff.
Keep hands, feet, and belongings to themselves.
Wear appropriate dance attire and shoes.
Use encouraging language.
Help create a safe and welcoming environment.
Care for studio equipment and property.
Repeated unsafe behavior may result in dismissal from class.
Dress Code
Students should arrive ready to dance or be able to change promptly wearing appropriate attire for each class they are enrolled in.
Recommended attire includes:
Dance leotard, fitted athletic clothing, or class-appropriate attire
Required dance shoes for the enrolled class (purchasing & renting options are available)
Hair secured away from the face (bun's preferred but pony tails are acceptable for classes, just not performances)
No gum, excessive jewelry, clothing that restricts movement, or loose revealing clothing
Personal Property
The studio is not responsible for lost, stolen, or damaged personal belongings.
Please label all shoes, water bottles, jackets, and dance bags.
Photography & Social Media
The studio may photograph or record classes, performances, and events for educational and promotional purposes. You can specify if you'd like to opt-out of this, however, photographs and video's taken during performances by other families isn't regulated.
Attendance & Make-Up Classes
Regular attendance helps students progress and is especially important 2 months prior to recital month to learn routines. Tuition is not refunded for any missed classes. Makeup classes may be offered when available but cannot be guaranteed.
Weather & Emergency Closures
Classes may be canceled due to severe weather, emergencies, or unforeseen circumstances. Families will be notified as soon as possible using the studio's preferred communication methods.
Performances
Students participating in performances may be required to attend rehearsals, wear designated costumes, and follow performance guidelines. Additional fees may apply for costumes or other optional events.
Acknowledgment
By signing below, I acknowledge that:
I have read this entire agreement.
I understand the risks involved in dance participation.
I voluntarily accept these risks.
I agree to follow all studio policies and procedures.
I understand that policies may be updated periodically.
I have had the opportunity to ask questions before signing.

Studio Policy Changes
Roots of Knowledge Academy & Creative Arts Studio LLC reserves the right to update studio policies as needed. Families will be notified of significant policy changes.
