Enrollment in Westchester Ballet Company programs is subject to our policies, which you will review and accept during registration.

I, the parent or legal guardian of the dancer being enrolled, acknowledge that participation in ballet classes, rehearsals, and performances involves inherent physical risks, including but not limited to muscle strains, sprains, falls, and other injuries. I understand these risks and voluntarily allow my child to participate in the instructional programs of Westchester Ballet Company (WBC).

In consideration of my child's enrollment, I agree, on behalf of myself, my child, and our heirs and assigns, to release, waive, and hold harmless Westchester Ballet Company, its board of directors, officers, employees, faculty, volunteers, and affiliates from any and all liability, claims, demands, or causes of action, including those arising from the ordinary negligence of WBC, arising out of or related to my child's participation in WBC's instructional programs, including injuries sustained during classes, rehearsals, performances, or related activities on or off WBC premises, except in cases of gross negligence or willful misconduct.

In the event of a medical emergency, I authorize WBC staff to obtain emergency medical treatment for my child if I cannot be reached, and I accept responsibility for any resulting medical expenses.

My electronic acceptance of this waiver constitutes my legal signature.

I, the parent or legal guardian of the dancer being enrolled, grant Westchester Ballet Company ("WBC") permission to photograph, film, and record my child during classes, rehearsals, performances, and related activities, and to use these images and recordings in WBC materials, including but not limited to its website, social media accounts, newsletters, printed programs, promotional materials, and educational content.
I understand that these images and recordings may be used without compensation, that WBC retains ownership of all such materials, and that this authorization remains in effect unless revoked by me in writing. I understand that my child's name will not be used without additional permission, and that all materials will be used in a respectful and appropriate manner.
