Participation Waiver & Liability Release
Program Ages: 4–18 years old
Practice Location: TNT Gymnastics, Hagerstown, Maryland (exact address on file)
1. Assumption of Risk
Participation in cheerleading includes physical activity such as tumbling, jumping, stunting,
stretching, and conditioning. I acknowledge that injuries may occur including but not limited to
sprains, fractures, falls, and other physical harm.
2. Medical Insurance Requirement
All students must maintain active medical insurance coverage to participate in PCR. PCR does not
provide medical insurance.
3. Medical Emergency Consent
I authorize PCR staff to obtain emergency medical treatment for my child if necessary. I understand
I am responsible for all related medical costs.
4. Liability Release
PCR, its owners, coaches, staff, volunteers, and facility partners (including TNT Gymnastics) are
not liable for injuries, accidents, or incidents occurring during practices, performances, or events.
5. Code of Conduct
Students must follow all rules, show respect, and avoid bullying, profanity, or disruptive behavior.
PCR reserves the right to remove any student at any time for misconduct.
6. Parent Conduct Policy
Disrespectful, threatening, or disruptive behavior by parents/guardians toward staff may result in
immediate removal of the student from the program.
7. Attendance & Removal Policy
Students may be removed from the program after 5 consecutive days of non-payment or repeated
policy violations.
8. Media Release
PCR may use photos/videos of students for promotional purposes unless a written opt-out is
provided.
9. Transportation & Facility Liability
PCR is not responsible for transportation to or from practices or events and is not liable for incidents
occurring outside official practice time.
10. No Refund Policy
All fees and tuition payments are non-refundable.
