Release of Liability
As the legal parent or guardian, I understand that dance is a physical activity that involves jumping, turning, kicking, rotating, and full-body coordination. I acknowledge that participation in dance classes, rehearsals, workshops, and performances carries inherent risks, including but not limited to: falls, sprains, strains, broken bones, and other injuries. I hereby release, waive, discharge, and hold harmless Ignite Dance Collective Inc., its owners, instructors, employees, guest artists, and representatives from any and all claims, liabilities, demands, and causes of action whatsoever, arising out of or related to any injuries, accidents, illnesses, or damages (to person or property) that may occur while on premises, under the supervision of Ignite Dance Collective Inc., or en route to or from any such premises.
Medical Emergencies
The undersigned permits Ignite Dance Collective Inc., its owner, and operators to seek medical treatment for the participant in the event they are unable to reach a parent or guardian.
Loss & Damage
I understand that Ignite Dance Collective is not responsible for lost, stolen, or damaged personal items brought to the studio, including but not limited to phones, dancewear, costumes, or valuables.
Payment Policy
I acknowledge that all class, costume, competition, and other related fees must be paid in full by the communicated deadlines. Late payments may be subject to additional charges. I understand that failure to pay may result in suspension from classes or events. No refunds will be issued once registration and payment are confirmed, as space in programs is limited.
Photo/Video Release
As the legal parent or guardian, I give permission to Ignite Dance Collective Inc. to use any photos or video footage of my child for promotional purposes, including but not limited to social media, the website, and marketing materials.
Acknowledgement
By signing below, I agree to all terms and conditions outlined above. I acknowledge that I have read and understood this waiver in its entirety and accept full responsibility for participation in any and all activities at Ignite Dance Collective. If I am signing on behalf of a participant under 18, I certify that I am their legal parent or guardian.
