I, as the participant or the parent/legal guardian of the minor participant, acknowledge that dance instruction and related activities involve inherent risks, including but not limited to physical exertion, slips, falls, collisions, contact with other participants or instructors, muscle strains, sprains, fractures, and other injuries, whether occurring at or away from the studio. I knowingly and voluntarily assume all such risks.
To the fullest extent permitted by Florida law, I release, waive, and agree to hold harmless Masterpiece Ballroom LLC (d/b/a Masterpiece Dance Complex), its owners, instructors, agents, and representatives, as well as Etudes Events LLC and J.D. Burt Company, Inc., from any and all claims, demands, or causes of action arising out of or related to participation in any programs, classes, events, or activities, and I agree to indemnify the released parties against any claim brought by me or on my behalf, including reasonable attorneys' fees.

NOTICE TO THE MINOR CHILD'S NATURAL GUARDIAN: READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF MASTERPIECE BALLROOM LLC USES REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY SIGNING THIS FORM YOU ARE GIVING UP YOUR CHILD'S RIGHT AND YOUR RIGHT TO RECOVER FROM MASTERPIECE BALLROOM LLC IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM, AND MASTERPIECE BALLROOM LLC SHOULD NOT REQUIRE YOU TO SIGN THIS FORM.

I certify that the participant is physically and mentally fit to participate in the registered classes and that I have medical coverage or personal means to cover expenses related to any injury. I authorize Masterpiece Ballroom LLC, its instructors, and designated adults to secure emergency medical treatment for the participant if I or my designated representative cannot be reached, and I accept financial responsibility for such treatment. I understand and consent that manual adjustment of a student's body to correct alignment and technique is a standard and strictly instructional practice in dance education. If any provision of this waiver is held unenforceable, the remaining provisions remain in full effect. This waiver is governed by the laws of the State of Florida.

I grant Masterpiece Ballroom LLC (d/b/a Masterpiece Dance Complex) the irrevocable, perpetual, royalty-free right to photograph, record, and use my or my child's image, likeness, and voice in its website, social media, publications, and promotional or instructional materials, in any media now known or later developed, without compensation. I waive any right to inspect or approve such materials and agree to make no claim of any kind against Masterpiece Ballroom LLC arising from their use.

A nonrefundable registration fee of $50 per participant is due upon registration and renews annually each August at the start of the dance season. Tuition is due on the 1st of every month; tuition not received by the 5th incurs a $25.00 late fee. Declined payments incur a $25.00 declined-payment fee.
IMPORTANT: Tuition is calculated across the full ten-month dance season (August–May) and averaged into ten equal monthly payments. Tuition does not increase for five-week months and does not decrease for holiday-shortened months. One monthly price, one monthly payment.
Masterpiece Dance Complex maintains a NO REFUND, NO CREDIT policy. Tuition and fees are nonrefundable.
Withdrawal: To withdraw a student, written notice must be received via the studio portal or studio email before the 1st of the month. The payment method on file will continue to be charged each month until written withdrawal confirmation is received. Non-attendance does not constitute withdrawal.
Payment methods: Payments by credit card incur a 3.0% processing surcharge, disclosed at checkout, which does not exceed our cost of acceptance. Payments by debit card or ACH/direct bank draft incur no surcharge. ACH is our recommended payment method.
