LIABILITY WAIVER FORM

I/we realize that participation in dance classes and activities could involve some possible bodily injury. Despite reasonable precautions being executed, accidents and injuries may occur. By signing this release form, I/we (the dancer and/or parent/guardian) assume all risks related to the use of any spaces used by Odyssey Dance Company LLC.

I/we agree to release and hold harmless Odyssey Dance Company, including its
teachers, instructors, dancers, staff members, volunteers, board and advisory members, and facilities used by the entity, from any cause of action, claims, or demands now and in the future. I/we will not hold Odyssey Dance Company, Inc. liable for any personal
injury or any personal property damage, which may occur on the premises before, during, or after classes and performances.

Furthermore, I/we agree to adhere to the class and facility rules, and assume full responsibility for my/our behavior in addition to
any damage I/we may cause to the facilities utilized by Odyssey Dance Company. I understand that Odyssey Dance Company is a licensed and insured organization. In the event that I/we should observe any unsafe conduct or conditions before, during, or after my/our classes and performances, I/we agree to report the unsafe conduct or conditions to the Owners and Directors as soon as reasonably possible.

I understand that I must choose to opt out of photos to ensure my child will not be posted online or in print. I understand that if I do not opt out, I permit Odyssey Dance Company to publish photographs and/or videos taken of me and/or my minor child during participation in classes, programs, performances, and events in printed materials, social media, and website. I acknowledge that I/we will not receive financial compensation or rights of ownership of any media posted or obtained by Odyssey Dance Company.

I understand that whether I opt out of photo publishing or not, my dancer is still required to attend picture day. If a family opts out of photo publishing, a group photo will be taken with and without that dancer.

If a dancer is present in a photo the studio chooses to use, that student will be blurred or blocked before posting.
