By registering myself or my child for classes, camps, events, or other activities at Precision Dance Company, I understand that participation in dance and physical activities involves inherent risks, including the possibility of injury.

I voluntarily assume these risks and agree to release and hold harmless Precision Dance Company, its owners, instructors, employees, and representatives from liability for injuries, accidents, loss, or damages arising from participation, except where prohibited by law.

I authorize Precision Dance Company staff to obtain emergency medical assistance when reasonably necessary if I cannot be reached. I understand that I am responsible for any resulting medical expenses.

By completing registration and acknowledging this waiver, I confirm that I have read, understand, and agree to these terms for myself and/or my participating child.
