In case of an emergency situation, I give Studio 32 Dance Company and employees permission to seek immediate medical attention for the previously named child, and I will not hold Studio 32 Dance Company responsible for said action.

I accept responsibility for any and all medical expenses incurred if emergency medical attention is required.

I give permission for myself and/or my child to be photographed and/or videotaped during classes, competitions, rehearsals, recital, and/or other performances by Studio 32 Dance Company for use in advertising, social media promotions, and/or as a learning tool.
