You must be over 18 years old to register for an account.

All policies and waivers must be agreed to and signed prior to taking any classes or participating in any activities of Ballare Dance Studio, LLC.

All student information obtained by Ballare Dance Studio, LLC, its employees, contractors, owners, and agents is stored in our database and will not be shared with any third parties. We may use your personal information to send you periodic communications by text message, e-mail, and regular mail to keep you up to date on current events at Ballare Dance Studio and new programs, products, or other opportunities that we may offer.

We do not sell any personal information to any third parties.

Except for the very limited purposes discussed in this paragraph, we also will not share any of your personal information with third parties. We may sometimes use other businesses to perform certain services for us, such as processing credit card payments, delivering our merchandise, sending postal mail, and providing marketing assistance and data analysis. We may disclose personal information to those businesses when necessary for them to complete a requested transaction or otherwise perform their duties. Those businesses will not be permitted to use your information for any purposes other than to perform their services for Ballare Dance Studio.

We do reserve the right to disclose visitor information in special cases when we have reason to believe that disclosing this information is necessary to identify, contact, or bring legal action against someone who may be causing injury to or interfering with (either intentionally or unintentionally) our rights or property, other visitors, or anyone else who could be harmed by those activities. This includes our right to exchange information with other organizations to protect against fraud and reduce credit risk. We also reserve the right to disclose visitor information when we believe in good faith that the law requires it. Customer information is generally one of the business assets involved in a business sale. In the event that Ballare Dance Studio, LLC or all of its assets were acquired, customer information would be among the assets transferred.

Parent, Guardian, or Adult Student: In consideration of the benefits derived from Ballare Dance Studio, LLC (Ballare), I do hereby agree to indemnify and hold harmless, release, and discharge Ballare and its staff from any and all claims of personal injuries or property loss or damages sustained by me/my child while participating in activities with Ballare and its staff. I authorize emergency first aid care for myself/my child in the event that I/(s)he becomes ill or injured while attending Ballare activities. I further authorize Ballare and its staff to retain the services of an MD or other qualified medical personnel to treat me/my child in the event of a medical emergency. I agree to inform Ballare in writing of any physical limitations and/or medical conditions that l/my child has, whether or not those conditions restrict my/my child's full participation. I/My child may decline to participate in any Ballare activity with prior written notice of at least 30 days to Ballare.

Filming and/or photography of any sort are not permitted at any time in any dance class or lobby without the studio owner’s permission. The studio is hereby granted permission to take photographs/videos of the students to use in brochures, websites, posters, advertisements, and other promotional materials the studio creates. Permission is also hereby granted for the studio to copyright such photographs/videos in its name.

I understand that I may opt out of this policy at any time by selecting the opt-out checkbox in the Waivers section of my account.

• Any pro-rated tuition and registration fees are due at the time of enrollment.
• Tuition is due on the 1st of each month.
• All registration fees, tuition, and private lessons are non-refundable.
• Tuition remains the same regardless of attendance or the number of weeks in the month.
• Tuition does not include costume fees, recital fees, or other special event costs.

• Payments can be made using debit cards, credit cards, or ACH (e-check).
• Credit card payments include a 3% processing fee, and ACH (e-check) payments include a 1% fee. Debit cards do not incur any processing fees.
• For your convenience, we offer autopayment options through our secure online portal.
• Cash payments are accepted and must be handed directly to Ms. Krystal.
• Payments by check are not accepted.

• A $20 late fee will be applied per family to any outstanding balance after the 7th of the month.
• If the balance is not paid by the 15th, students will not be able to participate in classes until the balance is paid.
• An insufficient funds/returned payment fee of $35.00 will be applied for any failed transactions.

• Makeup classes will NOT be offered for missed classes or inclement weather unless deemed appropriate by Ms. Krystal.
• Tuition is NOT adjusted because of unattended lessons. There are NO refunds or tuition reductions for missed classes.

• Students must provide at least 30 days’ written notice to withdraw from any class.
• Tuition will continue to be charged during this 30-day period, regardless of attendance.
• Refunds are NOT issued for missed classes or partial months, and costume, competition, or other prepaid fees are non-refundable once orders have been placed.

• Students must notify the studio 24 hours before an already scheduled private lesson to reschedule.
• Students canceling a private lesson less than 24 hours before the scheduled time of the lessons are responsible for the full lesson fee.

The following code of conduct applies to all visitors in the studio, whether student, parent, relative, sibling, or friend:
• NO visitors (i.e., parents, siblings, etc.) are allowed in the dance studio while class is conducted. Visitors may wait in their car, the dance studio lobby, or a designated waiting room until the end of class.
• NO food, drinks, candy, or gum is allowed in the studio. Only bottled water is allowed.
• Small children should NOT be left unattended. Please do not allow children to play with anything in the studio that does not belong to them.
• NO running and disruptive horseplay in the studio or waiting areas.
• Any student who continually disrupts a class may be dismissed from that class. If the behavior does not improve, the student may be permanently withdrawn from the class (with no refund).
• Ballare Dance Studio, LLC reserves the right to bar any person from entering due to disorderly conduct/disruptive behavior or for any other reason determined by management as unsuitable behavior.

• Students should arrive 5-10 minutes early to prepare for class.
• If you must leave, please return promptly to pick up students at the end of class. Ballare Dance Studio, LLC is not responsible for students left unattended.
