Assumption of Risk
I understand that participation in dance, gymnastics, tumbling, cheer, and related physical activities at Studio V involves inherent risks of injury, including but not limited to sprains, strains, fractures, and in rare cases, serious injury or death. I voluntarily assume full responsibility for these risks, whether foreseen or unforeseen.

Release of Liability
I hereby release, waive, discharge, and hold harmless Studio V, its owner, instructors, staff, contractors, volunteers, and affiliates from any and all liability, claims, demands, or cause of action that may arise from injury, illness, or accident involving my child’s participation in classes, open gym, performances, competitions, or any studio-related activity, whether on or off the premises.

Medical Treatment Authorization
In the event of an emergency, I authorize Studio V staff to obtain medical treatment for my child as deemed necessary. I understand that I am financially responsible for any medical expenses incurred.

Parental Responsibility
I confirm that my child is in good physical health and capable of participating in all enrolled activities. I agree to notify the studio of any medical conditions, injuries, or concerns that may affect participation. I also agree to comply with all studio rules, policies, and safety procedures.
