Assumption of Risk

I understand that participation in dance classes, rehearsals, performances, competitions, camps, workshops, acrobatics, tumbling, conditioning activities, and related programs offered by M.C. Dance Collective involves inherent risks of injury.

These risks may include, but are not limited to, slips, falls, collisions with other participants, strains, sprains, fractures, muscle injuries, head injuries, and other physical injuries that may result from participation in dance and related activities.

I voluntarily choose to participate, or allow my child to participate, in these activities and knowingly assume all risks associated with such participation.

Health and Physical Condition

I certify that I, or my child, am physically capable of participating in dance activities and have no medical condition that would prevent safe participation.

I agree to inform M.C. Dance Collective of any medical conditions, allergies, injuries, or limitations that may affect participation.

Medical Authorization

In the event of an emergency, I authorize M.C. Dance Collective staff to obtain emergency medical treatment for myself or my child if I cannot be reached immediately.

I understand that I am responsible for all medical expenses incurred as a result of treatment.

Release and Waiver of Liability

To the fullest extent permitted by law, I, on behalf of myself, my child, my heirs, personal representatives, and assigns, release and discharge M.C. Dance Collective, its owners, directors, instructors, employees, volunteers, contractors, and agents from any and all claims, demands, causes of action, damages, losses, or liabilities arising out of or related to participation in studio activities, except to the extent caused by willful misconduct or conduct that cannot legally be waived under applicable law.

I understand that this release applies to injuries, damages, or losses that may occur during classes, rehearsals, performances, competitions, studio events, and while present on studio premises.

Property Damage

I understand that M.C. Dance Collective is not responsible for lost, stolen, or damaged personal property brought onto studio premises.

Photography and Video Release

I grant permission for M.C. Dance Collective to photograph, record, and use images or video of myself or my child for promotional, educational, advertising, social media, and marketing purposes without compensation.

Studio Rules

I agree that I, or my child, will follow all studio policies, safety rules, instructor directions, and codes of conduct established by M.C. Dance Collective.

I understand that failure to follow studio rules may result in removal from class or activities.

Acknowledgment

I have carefully read this Participant Agreement, Assumption of Risk, and Liability Waiver. I understand its contents and sign it voluntarily.
