Liability

I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all rights and/or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Elite Dance Company Wa LLP, it's officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as "Elite Dance Company").

I hereby agree to release Elite Dance Company and hold Elite Dance Company harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant.

I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.

I have received or will obtain a copy of Elite Dance Company policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Elite Dance Company does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child's participation with Elite Dance Company, and I voluntarily assume the risks associated with such participation.

If I am a minor, my parent and/or legal guardian has also signed this document releasing Elite Dance Company from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity.

The participant has my permission to participate in Elite Dance Company. I release Elite Dance Company of all liabilities associated with my child's attendance at Elite Dance Company.

Tuition:

I understand that tuition is due by the first of every month. I understand that there will be a late fee charged if I do not pay by the 10th of the month, every month. I have had time to fully read and understand all tuition guidelines below and agree to comply with everything stated.

Tuition Guidelines:

Tuition is due by the 1st of the month. A late fee of $25 will be assessed on the 11th of the month for all delinquent accounts. There will be a fee of $35 for all returned checks.

A yearly registration fee of $25/child or $40/family is assessed in September or with your first month of tuition when you register September through June.

If you sign up after the first week of classes for the month, your tuition will be prorated for the remainder of the month. However, if you drop a class in the middle of the month there will be no refunds or credits given.

Tuition is based off of a twelve month calendar year. You have the option to pay for the entire year all at once or to have it divided into twelve equal monthly payments.

There will be dates and times that the studio is closed. We do not prorate for dates & times the studio is closed.
We do not refund for missed classes. You are always welcome to schedule a make-up class if you have missed. Please see the front desk to discuss your options.

We do not refund for cancelled classes. We do everything in our power to keep this from happening, but sometimes this is out of our control. If we cannot find a sub and the class must be cancelled, you are always welcome to schedule a make-up class. Please see the front desk to discuss your options.

Media:

I understand that Elite Dance Company offers recreational activities. Many families choose to take photos during these classes. At times, I also understand that Elite Dance Company will take pictures/videos for promotional purposes. I hereby give Elite Dance Company permission to use my child's picture and/or video in or on any form of advertisement for Elite Dance Company or an Elite Dance Company affiliated event, including their website and social media pages. I understand that pictures may be taken during classes or events and I agree to release my child's photos to Elite Dance Company for use.
