You must be over 18 to register for an account.

I, the undersigned participant and/or parents or legal guardian (if the participant is under 18 years of age), understand that participation in activities at THE DANCE BARRE CO. including recreational classes, competition team, rehearsals, performances, workshops, master classes, conventions, camps, private lessons, competitions, lock-ins, studio parties, banquet and all related dance activities, involves inherent risks of injury.

I, the undersigned participant certify that the participant is physically able to participate and agree to notify the Studio of any medical conditions, injuries, allergies, or limitations. In the even of an emergency medical treatment, I authorize THE DANCE BARRE CO. to obtain emergency medical assistance if I cannot be reached and accept full responsibility for any related medical expenses.

I, the undesigned participant voluntarily assume all risks associated with the participation, including but not limited to falls, strains, sprains, fractures, concussions, illness, permanent injury, or other unforeseen accidents that may occur on or off Studio premises.

To the fullest extent permitted by law. I release and hold harmless THE DANCE BARRE CO., its owners, directors, instructors, staff, guest artists, volunteers, and representatives from any claims, liabilities, damages, or expenses arising from participation in Studio activities, except where prohibited by law.

I, the undersigned participant understand that THE DANCE BARRE CO. is not responsible for lost, stolen, or damaged personal belongings and does not provide medical insurance for participants.

I, the undersigned participant agree to follow all Studio policies, safety rules. and instructor directions. Failures to do so my result in dismissal from activities without refund.


By submitting this agreement you acknowledge as the undersigned participant and/or parent or legal guardian (if the participant is under 18 years of age), that you have read and understand the Liability Waiver and voluntarily agree to its terms.

I, the undersigned participant and/or parent or legal guardian ( if the participant is under 18 years of age), understand that THE DANCE BARRE CO. may photograph, videotape, or record participants during classes, rehearsals, performances, competitions, workshops, camps, master classes, studio events, and other Studio-related activities.

I, the undersigned participant grant THE DANCE BARRE CO. and its authorized representatives permission to use these photographs, videos, and recordings for promotional, educational, advertising, marketing, social media, website, print, digital, and other business purposes without compensation or additional approval.

I understand that all photographs, videos, and recordings become the property of THE DANCE BARRE CO. and may be edited, reproduced, published, or distributed at the Studio's discretion.

I, the undersigned participant release and hold harmless THE DANCE BARRE CO., its owners, directors, employees, instructors, photographers, videographers, and representatives from any claims arising from the use of theses images or recordings, including claims relating to privacy, publicity, or copyright, to the fullest extent permitted by law.

If I do not wish for the participant's image or likeness to be used, I understand that I must submit a written request to THE DANCE BARRE CO. before participation. While the Studio will make reasonable efforts to honor such requests, it cannot guarantee exclusions from group photographs or recordings taken public event or performances.

By submitting this agreement you acknowledge as the undersigned participant and/or parent or legal guardian (if the participant is under 18 years of age), that you have read and understand the Photo & Video Release Agreement and voluntarily agree to its terms.
