ELECTRONIC COMMUNICATION- We communicate electronically with our families. Parents must have an active email address or be willing to receive studio text messages.
Drop off and pick up- Students should be dropped off and picked up no sooner than 10 minutes before class if possible and picked up immediately after class to avoid congestion in the lobby.
No drinks or food allowed in any of the studio rooms. Only water bottles allowed!
No street shoes in the studio rooms please leave them in the lobby or carpeted areas
Registration form and waiver must be signed and turned in at the first attendance class
Tuition - Our dance season is 9 even payments- due the first of each month, running September 1st through May 1st. Eft and Credit card payments will be charged on the 1st of each month. Class fees are only refundable with a 30 day notice. Once payments have been charged they are non refundable. If you choose to pay for the year in full, there are NO refunds if your dancer decides to stop mid season. If you think this might be a possibility please sign up for monthly payments. We do not give credit/refunds for classes missed due to holidays, illness, vacations or weather.
Costume payments - Costume fees will be due October 15th! All eft and credit card payments will be charged on the 15th of October. Once costumes have been ordered they are non refundable
Late fees - A $20 late fee will be added to any payments that are 10 days late or more
CLASS CANCELLATIONS- In the event that a class is canceled due to an emergency or instructor illness, a makeup class will be scheduled. In the event of inclement weather, cancellation notifications will be sent via email, text message, and posted on our social media pages. Weather-related cancellations will not typically be made up. However, Dancers Unlimited reserves the right to schedule a makeup class if additional rehearsal time is needed to adequately prepare students for performances, recitals, or competitions.
LOST ITEMS: Dancers Unlimited, its teachers, and assistants are not responsible for lost or stolen items. Please send students dance attire and shoes in a dance bag. Make sure all shoes are labeled with their name inside.
Assumption of Risk- I, recognize and understand the risks of physical injury inherent to dance and dance training and hereby acknowledge that I am aware that there are significant risks associated with participation in such dance classes, tumbling classes, rehearsals, and recitals including without limitation, the risk of serious bodily injury or death. I understand that it is my responsibility to consult with a physician prior to and regarding my participation or my child's participation in Dancers Unlimited classes, rehearsals and recitals. On behalf of myself, my child, I willingly assume such risks and I hereby release Dancers Unlimited, its teachers and assistants from any and all liability, claims, judgments or demands arising from injuries or illnesses contracted while attending class, rehearsal, recitals and competitions.
Medical Emergency - In the event of a medical emergency, and a parent or guardian is not reachable, I give Dancers Unlimited and its teachers permission to seek medical attention if deemed necessary.
Use of Photography & Videography - I grant permission for Dancers Unlimited to photograph, videotape, and/or record myself or my child during classes, rehearsals, performances, competitions, camps, and studio events. I understand that these images and recordings may be used for promotional purposes including social media, websites, advertising, marketing materials, and studio publications without compensation.
Release of Liability - In consideration of participation in activities at Dancers Unlimited, I, on behalf of myself and/or my child, hereby release, waive, discharge, and hold harmless Dancers Unlimited, Dancers Unlimited Dance Company, its owners, instructors, employees, contractors, volunteers, assistants, and facility partners from any and all claims, liabilities, demands, actions, damages, costs, or expenses arising from injury, illness, loss, or damage that may occur as a result of participation. This release includes claims arising from ordinary negligence but does not apply to gross negligence or intentional misconduct.
COMPETITION, PERFORMANCE & TRAVEL ACKNOWLEDGMENT - I understand that participation in competitions, performances, conventions, camps, workshops, community events, and other activities may require travel and participation in facilities not owned or controlled by Dancers Unlimited. I acknowledge that additional risks may arise during travel, lodging, performances, backstage activities, and participation at outside venues. I voluntarily assume all risks associated with participation in these activities and release Dancers Unlimited, its owners, instructors, employees, contractors, volunteers, and representatives from liability arising from participation in such activities.
TUMBLING & ACROBATICS ACKNOWLEDGMENT - I understand that tumbling, acrobatics, aerial skills, flexibility training, strength training, and related activities involve a higher degree of physical risk than traditional dance instruction. I acknowledge that injuries may occur despite proper instruction, spotting, supervision, and safety equipment. I understand that students are expected to perform only skills that have been approved by their instructor and agree not to attempt skills beyond their current ability level without instructor supervision. I voluntarily assume all risks associated with participation in tumbling and acrobatic activities.
COMMUNICABLE ILLNESS ACKNOWLEDGMENT - I understand that participation in dance classes, rehearsals, performances, competitions, camps, and other group activities may expose participants to communicable illnesses, including but not limited to influenza, COVID-19, RSV, and other viral or bacterial infections. I voluntarily assume the risk of exposure and agree not to hold Dancers Unlimited liable for illness that may occur as a result of participation. I agree to keep my child home when experiencing symptoms of illness that may place others at risk.
EQUIPMENT & FACILITY USE ACKNOWLEDGMENT - I understand that participation may involve the use of dance floors, tumbling mats, gymnastics equipment, portable flooring, props, costumes, sound equipment, and other studio property. I agree to use all equipment as instructed and understand that improper use may result in injury. I assume all risks associated with the use of studio equipment and facilities.
Community Performances & Public Events - I understand that dancers may be invited to participate in community performances, parades, exhibitions, promotional events, demonstrations, and other public appearances sponsored or organized by Dancers Unlimited. I acknowledge that participation in these events is voluntary and may involve travel, outdoor environments, unfamiliar performance surfaces, and interaction with the public. I voluntarily assume all risks associated with participation in these events and release Dancers Unlimited from liability arising from such participation.
Summer Camps & Special Events Acknowledgment - I understand that participation in summer camps, workshops, intensives, clinics, master classes, guest instructor programs, and special events may involve increased physical activity and extended periods of participation. I voluntarily assume all risks associated with participation in these programs and release Dancers Unlimited from liability arising from such participation.
Studio Policies Acknowledgment - I acknowledge that I have read and understand the policies and expectations of Dancers Unlimited. I agree to follow all studio rules and safety guidelines and understand that failure to do so may result in removal from activities without refund.

I understand that participation in activities offered by Dancers Unlimited may include, but is not limited to:
Dance classes
Year-round tumbling classes
Acro and conditioning classes
Summer camps and workshops
Private lessons
Rehearsals
Community performances and appearances
Recitals and showcases
Competitions and conventions
Special events and studio-sponsored activities
I understand that these activities may take place at any Dancers Unlimited location, including Spirit Lake, Iowa; Blue Earth, Minnesota; Algona, Iowa; or at off-site facilities, performance venues, competition locations, convention centers, schools, parks, or other event locations.
I acknowledge that participation in any of these activities involves inherent risks of injury and voluntarily assume all such risks.
ELECTRONIC SIGNATURE CONSENT
