Behavior Expectations & Studio Code of Conduct
PowerWorx Dance is committed to providing a positive, safe, inclusive, and respectful environment for all students, families, instructors, and staff. Students and parents are expected to behave respectfully and follow all studio policies. PowerWorx Dance reserves the right to suspend or dismiss any student or family whose conduct is unsafe, disruptive, or detrimental to the studio community. No refunds will be issued due to suspension or dismissal resulting from policy violations.

Emergency Medical Authorization
I authorize PowerWorx Dance staff to obtain emergency medical treatment for myself or my child if I cannot be reached immediately.

Liability Waiver & Release of Claims
I acknowledge the risks associated with participation in dance classes, rehearsals, performances, competitions, camps, workshops, acro, tumbling, conditioning programs, and related activities. I voluntarily assume all risks and release PowerWorx Dance, Frye Force Dance LLC, and its representatives from claims arising from participation, except where prohibited by law.

Photo & Video Release
YES, I grant permission for myself/my child to be photographed and/or recorded for promotional, marketing, website, and social media purposes.

Studio Policy Acknowledgment
I acknowledge that I have received, read, and agree to comply with all PowerWorx Dance policies, including tuition, attendance, withdrawal, recital, costume, and competition policies.
