I/we realize that participation in dance classes and activities involves inherent risks, including but not limited to bodily injury, illness, falls, physical exertion, and other unforeseen risks. Despite reasonable precautions taken by The Dance Collective, accidents and injuries may occur. By signing this waiver, I/we voluntarily assume all risks associated with participation in activities conducted by The Dance Collective and the use of any facilities or spaces utilized by the organization.

I/we agree to release and hold harmless The Dance Collective including its teachers, instructors, dancers, staff members, volunteers, board and advisory members and facilities used by the entity from any cause of action, claims, or demands now and in the future. I/we will not hold The Dance Collective liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes and performances. Furthermore, I/we agree to adhere to the class and facility rules and assume full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by The Dance Collective.

I/We authorize The Dance Collective and its representatives to obtain emergency medical treatment for the participant if deemed necessary. I/We understand that I/we are responsible for any costs associated with such treatment.

I understand that The Dance Collective maintains appropriate insurance coverage. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes and performances, I/we agree to report the unsafe conduct or conditions to the Artistic Director, Executive Director, instructor or a staff member as soon as reasonably possible.

I acknowledge that I/we will not receive financial compensation or rights of ownership.

By signing below, I/we acknowledge that I/we have carefully read this Liability Waiver and Release, fully understand its contents, and voluntarily agree to its terms.

I give permission to The Dance Collective to publish photographs and/or video taken of me and/or my minor child during participation in classes, drop-in classes, performances and events in The Dance Collectives printed materials, social media pages and website.

Monthly Tuition:
Annual registration fee of $25.00 will be due upon completion of class enrollment
Payment is due on the 10th of each month and covers that months tuition
No envelopes or statements will be handed out or mailed. It is the families’ responsibility to provide payment to the studio.
ALL ENROLLMENTS ARE REQUIRED TO BE ON AUTOPAY. Tuition may be paid via debit/credit card. Cash and check payment can be accepted by the 5th of the month.
Tuition is based on a full year fee and not by the individual class or weeks, Some months may have 2 classes and some may have 3, 4, or, 5. All planned studio closures have been taken into consideration when setting tuition pricing.
Accounts not paid by the 10th of the month are subject to a $35.00 late fee.
Students will not be allowed to take class if their account is delinquent.
Monthly tuition payments are non-refundable and we do not offer reimbursement for classes missed

Drop Policy: classes may be dropped at any time throughout the session by notifying us in writing by the 9th of the month to discontinue charges to your account for the following month. Notification by email is preferred. If notification is received after the 9th, your account will still be charged for the following month and not refunded.
A minimum of 4 students are required to keep a class open. All classes/instructors are subject to change.

Studio Rules for Dancers & Families:
Students who arrive more than 15 minutes late may be asked to observe class
Students who arrive more than 5 minutes late to class will be marked tardy
Be respectful to faculty, peers, and all TDC staff.
TDC will not accept bullying or harassment. I/we agree to report bullying/harassment to the Artistic Director, Executive Director, instructor or a staff member as soon as reasonably possible. TDC is committed to addressing and resolving any conflicts as promptly as possible. TDC holds to remove anyone from the studio and classes if bullying/harassment continues.
No gossip, obscene language or inappropriate language are tolerated during class or in the lobby.
No gum allowed in class
No cell phones allowed in class (Please keep all personal items in your dance bag).
If ill, do not come to class

Lost and Found:
TDC is not responsible for any lost or stolen items. Dancers should take their personal items into the dance studio with them and place them in cubbies. Any belongings left in the lobby will not be supervised.
